b. PhilGEPS Registration Number;
c. Valid Tax Clearance (per Executive Order 398, Series of 2005, as finally reviewed and approved by the BIR)
II. Original Copy of:
d. Conformity with the Technical Specifications (Attached as Annex A)
e. Financial Proposal Form (Attached as Annex B)
f. Notarized Omnibus Sworn Statement (Attached as Annex C)
