The Gambia has obtained considerable achievements in maternal and child survival. Between 1990 and 2013, the maternal mortality ratio declined significantly from 1050 to 433 per 100, 000 live births and under five mortality fell from 165 to 54 per 1, 000 live births. The country has reached its MDG4 target of reducing child mortality according to the DHS survey but failed to achieve the MDG target on maternal mortality.

The leading causes of maternal deaths are haemorrhage (34%), hypertensive disorders in pregnancy (19%), and sepsis (9%). Further evidence has revealed that maternal mortality was largely due to teenage pregnancy, limited access to skilled birth attendants (57.2%), and low quality of emergency obstetric and neonatal care services. Seven health facilities provide such comprehensive services. In addition, early marriage is common as 41% of women aged 20-49 are married by age 18, and contraceptive prevalence rate (cpr) is very low only at 9%, and unmet need for family planning is about 24.9%.

the leading causes of death in children under five are neonatal causes (40%), following malaria (23%), pneumonia (16%), and diarrhoea (14%). with an estimated mortality rate of 22 deaths per 1, 000 live births among neonates in 2013, neonatal mortality now accounts for a big share of all under-five deaths. it is estimated that every year in the gambia, about 2, 400 children die during their first 28 days, of which 816 (34%), 672 (28%), and 552 (23%) because of preterm, infection and asphyxia respectively. it is necessary to note that many of these neonatal deaths could be prevented by providing temperature stability, respiratory support, hydration and nutrition; preventing and treating infections, providing skilled staff, awareness creation; and diagnosing and treating neonatal jaundice and hypoglycaemia.

the ministry of health and social welfare with support from usaid, who, unfpa and unicef has embarked on assessment of health systems in the gambia. within this context, unicef is supporting the mohsw with assessment of health services. for this an international consultant is being hired. however, given high levels and rising trends of morbidity and mortality related with maternal and newborn health, it was agreed that within the wider context, more in depth analysis of maternal and new born care services needs to be carried out. in view of this it was agreed that a local consultant will be taken on board to specifically focus on these services and to assess what is required to provide quality maternal and newborn care. the assessment results will guide evidence based programme planning and long term monitoring on the provision of quality maternal and newborn care and the prevention of maternal and newborn deaths.

how can you make a difference?

under the general guidance of the maternal and newborn working group, close coordination with the unicef co and supporting the international consultant, the incumbent will be required to develop and implement assessment of selected health facilities around the country.

specifically, the incumbent shall:

· planning and develop a detailed the assessment tool for the selected health facilities with oversight and guidance from the ceman working group

· conduct the assessment of the selected health facilities

· assess the general policy environment as it relates to delivery of mch services (including guidelines and sops)

· assess the institutional capacity, and integration in relation to delivery of mch

· assess the quality of care with regards the delivery of mch services

· identify factors that contribute to facility maternal and newborn deaths.

· assess the facility for emergency referral of maternal and newborn complications.

· assess the service providers knowledge and competence on maternal and newborn care.

· assess the potential for the generation of relevant and timely mch service data

· disseminate information collected and recommendations to the ceman technical working group

· draft an implementation plan for review

· validate findings with selected stakeholders

· draft a report of the assessment in english based on the outcome of assessment

· review and finalize the draft report based on comments and feedback received from various stakeholders

· will support the international consultant in implementation of health services component in terms of liaison with mohsw and supervision of data collection process. data sharing of wider health services assessment will only be through the international consultant.

to qualify as an advocate for every child you will have…

· an advanced university degree (master’s or higher) in public health or it's equivalent*a first university degree in a relevant field combined with 2 additional years of professional experience may be accepted in lieu of an advanced university degree.

· the candidate should have background knowledge on maternal and child health issues and befamiliar with the health system especially maternity and neonatal units

· a minimum of 5 years of relevant professional experience and experience in carrying out similar exercises

· fluency in english is required. knowledge of a local language is an asset

· excellent organizational skills and the ability to lead a team

· excellent analytical and report writing skills.
