Original Request for Quotation form will be available for pick-up or download from PhilGEPS starting OCTOBER 16 until OCTOBER 22, 10:00 AM at the Procurement Management Division, Human Resource and Administrative Service, Ground Floor, Trade and Industry Bldg., 361 Sen. Gil Puyat Avenue, Makati City c/o Ms. Clarisse Anne D. Salenga at telephone # 751-4997, fax # 895-3515 or e-mail ClarisseAnneSalenga@dti.gov.ph. It is requested that the quotation be sealed, originally signed or stamped in an envelope. Sealed quotation should be DROPPED in our bid box located at the Procurement Management Division.
