Invitation for Negotiated Procurement Supply and Delivery of 1 Unit Ambulance Van

1. In view of the Two (2) failed biddings, the Municipality of Caraga through its Bids and Awards Committee, invites interested bidders to participate in the negotiation for the Supply and Delivery of 1 UNIT AMBULANCE VAN, with an Approved Budget for the Contract (ABC) of One Million Seven Hundred Thousand Pesos (PhP 1, 700, 000.00) in accordance with section 53.1 of the Implementing Rules and Regulations of Republic Act No. 9184, otherwise known as the -œGovernment Procurement Reform Act.-



For more details on this project, please refer to attached Schedule of Requirements (Annex A) and Technical Specifications (Annex B).



2. The following eligibility and technical documents as well as the Financial Proposal Form (Annex C) shall be submitted on or before November 25, 2019 at 12:00NN at the BAC Conference Room, Municipal Hall, Caraga, Davao Oriental:

a) Securities and Exchange Commission (SEC) / Department of Trade and Industry (DTI) Registration Certificate;

b) Valid Mayor-s Permit

c) Statement of all its ongoing and completed government and private contracts for the last 3 Years up to the day before the deadline of submission of bids, including contracts awarded but not yet started, if any. The statement shall include, for each contract, the following:

(1) name of the contract;

(2) date of the contract;

(3) kinds of Goods;

(4) amount of contract and value of outstanding contracts;







(5) date of delivery; and

(6) end user-s acceptance or official receipt(s) issued for the contract, if completed.

Bidders shall submit separate statements for (1) single largest completed contract; and (2) on-going and awarded but not yet started contracts.



d) Bid security shall be either in the following forms and amount:

i) 2% of the ABC, if cash or cashier-s/manager-s check, bank draft/guarantee or irrevocable letter of credit issued by a Universal or Commercial bank;

ii) 5% of the ABC, if surety bond with certification from Insurance Commission that such insurance company is authorized to issue such security;

iii) any combination of the foregoing; and

iv) Bid Securing Declaration. Sample form is attached as Annex D.

e) Compliance with the Schedule of Requirements (Annex A) and Technical Specifications (Annex B);

f) Sworn statement in accordance with section 25.2(a)(iv) of the IRR of RA 9184 and using the prescribed form. (Annex E)

g) Tax Clearance Certificate for 2013 per Executive Order No. 398 series of 2005.

h) Audited Financial Statements (AFS) for the last 2 years, stamped -œreceived- by the Bureau of Internal Revenue (BIR) or its duly accredited and authorized.

i) Authority of the Signatory; and

j) Certification of PhilGeps Registration.

3. The complete set of Bidding Documents for Negotiated Procurement may be purchased at the BAC Secretariat-s Office upon submission of a Letter of Intent (LOI) from the company and payment of a non-refundable fee in the amount of Five Thousand Pesos (PhP 5, 000.00) not later than November 22, 2019. Bidders who have been declared -œeligible- during the first two-failed biddings need not pay for the bid documents.

4. The bidders shall drop their duly accomplished Bid Proposals in sealed envelopes in the box located at the BAC Conference room, Municipal Hall, Caraga, Davao Oriental on or before 12:00NN of November 25, 2019.

Opening of the Eligibility Documents, Technical Documents and Financial Proposal will be on November 25, 2019 at 1:00PM at the BAC Conference Room, Municipal Hall, Caraga, Davao Oriental.

5. The Municipality of Caraga reserves the right to reject any offer, to annul the negotiation process, and to reject all offers at any time prior to contract award, without thereby incurring any in, without thereby incurring any liability to the affected firm/bidder or bidders.
