If you are a committed, creative professional and are passionate about making a lasting difference for children, the world's leading children's rights organization would like to hear from you.For 60 years, UNICEF has been working on the ground in 190 countries and territories to promote children's survival, protection and development. The world's largest provider of vaccines for developing countries, UNICEF supports child health and nutrition, good water and sanitation, quality education for all boys and girls, and the protection of children from violence, exploitation, and AIDS. UNICEF is funded entirely by the voluntary contributions of individuals, businesses, foundations and governments.

A) History and Background

At the 28th Summit of the African Union, African heads of state endorsed the Addis Ababa Declaration on Immunization (ADI). This commitment paves the way for accelerated efforts by governments to ensure equitable access to vaccines for people. For Guinea, this high-level commitment serves as a catalyst for ending the epidemics of vaccine-preventable diseases and accelerating the achievement of the Sustainable Development Goals by 2030. The goal is to achieve universal coverage for immunization and other related services including, vitamin A supplementation, Mebendazole deworming and birth registration.

From 2014 to 2015, Guinea's Ebola outbreak has had a negative impact on the supply and use of health services, including immunization. The results of the MICS conducted in 2016 corroborated this situation by revealing that only 26.3% of children aged 12 to 23 months received all the vaccines recommended by the PEV program nationwide. In some regions such as Labé and Mamou, these rates are 7% and 13.8% respectively. In addition, 25.3% of children in this age group received no dose of vaccine in the same period. The decline in CV observed, has been accompanied since 2015 by a resurgence of epidemics of diseases preventable by vaccination including: polio, measles, and whooping cough. This situation has also compromised

In its efforts to revive the routine EPI, in the context of strengthening primary health care, Guinea faces multiple challenges, including:

i) weak local governance and lack of accountability at the political, administrative and community levels;

ii) a lack of commitment and clear strategies for adequate and sustainable funding for immunization.

(iii) inefficient planning and management of human and technical resources;

(iv) inadequate convergence and integration of immunization efforts with other interventions both within and outside the health sector,

v) low involvement of communities in general and community-based associations in particular.

To improve the health status of the population and to ensure the accessibility and equity of all immunization services, the Guinean Government, in its sectoral health strategy, has set itself the goal of reducing morbidity and mortality. due to vaccine preventable diseases. To this end, it has adopted a comprehensive multi-year strategic plan (cMYP) for the period from 2016 to 2020, the objectives of which are aligned with those of the GVAP 2011-2020, the Health Development Plan (PNDS) 2015-2024 and of the 2015-2017 Health System Recovery and Resilience Plan. The Minister of Health recently convened a national forum to review progress in all areas of the EPI, to analyze weaknesses and

B) Justification:

Guinea has received the Gavi grant (2017-2019) for the strengthening of the health system (RSS2), including the vaccination system. Also, in mid-September 2018, the country will organize a joint review of its Expanded Program of Immunization to allow the capitalization of the strategic orientations of this review in the formulation of priority actions.

Bearing in mind that the coverage data from the MICS surveys indicate significant disparities between socio-economic groups with a decrease in performance between 2012 and 2016 for most indicators probably related to the Ebola outbreak, including:

· Immunization coverage rates in Penta 3 for boys and girls increased successively from 52.3% and 47% in 2013 to 41.4% and 37.6% in 2016

· Penta 3 coverage rates in urban and rural areas increased successively from 62.2% and 46.1% in 2013 to 54.5% and 31.4% in 2016

· Coverage rates in Penta 3 between the richest and the poorest rose successively from 62.2% and 32.4% in 2013 to 63.6% and 17.9% in 2016

· The Penta 3 coverage rates in relation to the education level of the mother according to whether she is educated or not went successively from 71.9% and 46.1% in 2013 to 73.3% and 352% in 2016

An analysis of equity at the level of the most decentralized unit is essential. Building on existing strategies for immunization planning and implementation, including the CDA approach, this analysis will consider alternative approaches with a focus on service delivery bottlenecks. of cold, financial management and review of the implementation of the 5 ACD components as well as a mapping of immunization activities. the exercise will cover all 8 regions of the country by considering a possibility of representation of all districts of the country.

To facilitate the implementation of this approach to the analysis of equity in immunization Guinea, the UNICEF office wants to recruit the services of an International Consultant
