PROCUREMENT OF HAULING/ TRANSPORTATION SERVICES FOR USE IN THE DELIVERY OF DRUGS AND MEDICINES, MEDICAL AND OTHER SUPPLIES & MEDICAL EQUIPMENT TO DIFFERENT RECIPIENTS IN REGION 8 FOR THE PERIOD NOVEMBER TO DECEMBER 2019 (ORDERING AGREEMENT)



1. The Department of Health - Eastern Visayas Center for Health Development through the General Appropriations Act (GAA) for FY 2019 intends to apply the sum of Two Million Three Hundred Nineteen Thousand Nine Hundred Ninety-Nine Pesos and Forty-Three Centavos Only (Php2, 319, 999.43) being the Approved Budget for the Contract (ABC) to payments under the contract for the Procurement of Hauling/ Transportation Services for Use in the Delivery Of Drugs and Medicines, Medical and Other Supplies & Medical Equipment to Different Recipients in Region 8 for the Period November to December 2019 (Ordering Agreement). Bids received in excess of the ABC shall be automatically rejected at bid opening.



2. Prospective bidders should have completed, within two (2) years from date of submission and receipt of bids, one (1) similar contract and the contract amount should be equivalent to at least 25% of the ABC. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders.



3. Bidding will be conducted through open competitive bidding procedures using non-discretionary -œpass/fail- criterion as specified in the 2016 revised Implementing Rules and Regulations (IRR) of Republic Act 9184 otherwise known as the -œGovernment Procurement Reform Act-.



Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA 5183.



4. Interested bidders may obtain further information from DOH-EV CHD, BAC Secretariat, Government Center, Palo, Leyte during Mondays through Fridays at 8:00 AM to 5:00 PM.

A complete set of Bidding Documents may be acquired by interested bidders from October 18, 2019 to November 6, 2019 from the address below and upon payment of the applicable fee for the Bidding Documents pursuant to the latest Guidelines issued by the GPPB, in the amount of FIVE THOUSAND PESOS (Php 5, 000.00).



It may also be downloaded from the website of the Philippine Government Electronic Procurement System (PHILGEPS) and the website of the DOH - Eastern Visayas Center for Health Development, provided that bidders shall pay the non-refundable fee for the Bidding Documents not later than the submission of their bids.

5. The DOH- Eastern Visayas Center for Health Development will hold a Pre-Bid Conference on October 25, 2019 at the DOH-EV CHD, HRTC, Government Center, Palo, Leyte, which shall be open to prospective bidders.



6. Bids must be duly received by the BAC Secretariat at the address below on or before the deadline of the submission of bids stated below. All Bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 18.



Bid opening shall be on November 6, 2019 at 9:30 AM at the DOH-EV CHD, HRTC, Government Center, Palo, Leyte. Bids will be opened in the presence of the bidders- representatives who choose to attend at the address below. Late bids shall not be accepted.



7. Attendance of the bidders shall not be mandatory in the Pre-Bid Conference.



8. After opening of bids, Members of the BAC, its staff and personnel, Secretariat and TWG, as well as Observers, are prohibited from making or accepting any communication with any bidder regarding the evaluation of their bids until the issuance of the Notice of Award. However, the BAC, through its Secretariat, may ask in writing the bidder for a clarification of its bid. All responses to request for clarification shall be in writing....
