Hauling/Delivery of first batch of Sari Sari Store Starter Kits from DTI-TFBM Warehouses to Marawi City and Iligan City

Hauling/Delivery of first batch of Sari Sari Store Starter Kits (48kg/box | L -80cm H-54cm W-59cm) from DTI-TFBM Warehouses to the following venues:

1. Within Marawi City 3, 070 boxes with ABC of P127.00 per box

2. Within, Iligan City 1, 000 boxes with ABC of P 52.00 per box

Activities include the ff:

- Labor services for loading from warehouse to vehicle and unloading from vehicle to distribution sites of Starter Kits;

- Assistance in securing the kits in the vehicle and during the distribution at distribution sites;

- Standby Trailer/vehicle until the completion of distribution of Kits - Backloading, in case of undistributed kits

- At least two (2) 40-footer container van to carry 200 boxes per van every distribution schedule in Marawi City

-Billing and Payment will be made in a weekly basis."

INCLUSIONS:

1) The bidders shall submit the following Eligibility Documents in addition to the prescribed quotation form:

(i) Registration certificate from the Securities and Exchange Commission (SEC) for corporations, Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, whichever is applicable.

(ii) Valid Mayor's permit issued by the city or municipality where the principal place of business of the prospective bidder is located.

(iii) Philgeps Registration Number

(iv) Latest Income Tax Return

(v) Omnibus Sworn Statement

2) All entries shall be typed or written in a clearly legible manner.

3) Prices are inclusive of taxes, delivery and other incidental costs.

4) The deadline for submission of quotation shall be on April 23, 2019 2:00PM. The bidders shall send their duly accomplished form together with the eligibility requirements in three copies to DTI-LANAO DEL NORTE PROVINCIAL OFFICE, 0171 PM Durias Bldg., Quezon Ave., Ext., Palao, Iligan City

The Notice of Award (NOA) shall be issued upon receipt of Notice of Cash Allocation (NCA).

DTI Lanao del Norte reserves the right to waive any required formality in the proposals received and select the proposal which it determines to be advantageous to the government.
