Goods 2018-10 - Technical Skills Training - Food and Beverages Servicing

REQUEST FOR QUOTATION

The Municipal Government of Cavinti, Laguna will undertake a Negotiated Procurement (Two-Failed Biddings) for Technical Skills Training - Food and Beverages Servicing (Goods 2018-10) which will be implemented by the LGU thru the OMSWDO, in accordance with Section 53.1 of the 2016 Revised Implementing Rules and Regulations of RA 9184. The details of the project are as follows:

Cavinti, Laguna

Approved Budget for the Contract:

Two Hundred Fifty Five Thousand Two Hundred Pesos Only (Php 255, 200.00)

Contract Duration:

Completion of Training shall be made within Twenty Five (25) days from the date of receipt of the Notice to Proceed (NTP).

Specifications:

The Technical Skills Training on Food & Beverages Servicing is for Twenty (22) persons to be trained within not less than Twenty Five (25) days. Includes the training cost, identification cards, forms TOR, certificate, uniform and assessment fee.

Interested suppliers are required to submit the following:

1. PhilGEPS Registration

2. Mayor's Permit

3. DTI or SEC Registration Certificate

4. Income/Business Tax Return

5. Omnibus Sworn Statement (Annex A)

6. Schedule of Requirements (Annex B)

7. Technical Specifications (Annex C)

8. Price Quotation (Annex D)

Submission of quotation and documentary requirements is on or before November 20, 2018, 10:00 a.m. at Office of the Municipal Assessor, Municipal Hall, Cavinti, Laguna.
