REQUEST FOR QUOTATION (RFQ)

The Municipal Government of Antipas, through its Bids and Awards Committee (BAC), hereby invites interested suppliers to submit price quotation for Fuel for Road Maintenance with the following items with ABC of P199, 950.00

Request for Quotation form can be obtained from BAC Secretariat, Municipal Hall, Antipas, Cotabato on August 20-23, 2019 and the deadline for submission of sealed quotation is on August 23, 2019 at 9:00 A.M.

Opening of sealed quotation is on August 23, 2019 at 9:00 A.M. at MPDO, Antipas, Cotabato.

Interested suppliers should submit their sealed quotations to the BAC Chairman, LGU-Antipas, Cotabato. Suppliers are required to submit the following:

a) Valid Business Permit

b) Philgeps Registration Certificate/Number

c) Omnibus Sworn Statement

The Municipal Government of Antipas reserves the right to accept or reject any quotations, to annul the Request for Quotation process, and to reject all quotations at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
