The subject of the order is: purchase and successive supply of xerographic paper in reams (1 ream = 500 sheets), weight 70-90 g / m, designed for one and two-sided printing in black and multi-color on copying, printing and multifunction devices, laser andramentowych divided into 2 parts.

Main Site: According to Annex No. 4 to the model contract.

Purchase and delivery of A4 paper in the number of 694, 102, - the Ordering Party does not allow the possibility of exceeding the quantity indicated. The Ordering Party reserves the right to reduce the volume of ordered photocopied paper in relation to the quantities indicated in the Distribution of deliveries, refueling Annex 4 to the Agreement by no more than 20%, and thus not using the entire Agreement amount. The Employer will inform the Contractor in writing of the above fact. Due to the above, the Contractor can not lodge any claims.

1. 7. Contractor's statement about belonging or not belonging to the same groupcapital dust (...) in accordance with - 5 item 10 of the above Regulation.

8. Declaration of the Contractor about non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716) in accordance with - 5 item 9 above Regulation.

III. Other additional information

1. The Contractor submitting the bid is obliged to pay a bid bond in the amount of: a) for part A - 245, 000.00 PLN

b) for part B - PLN 3 700.00.

2. If an appeal is lodged, the appellant shall send a copy of the appeal to the Employer by fax only to the number (22) 667-17-33 / 36, or by email only to the following address: SekretariatDZP@zus.pl

3. Changes to the provisions of the agreement dthey will be covered in accordance with the provisions of the ToR and model contracts

4. The Contractor remains bound by the offer for a period of 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires
