The subject of the order is the delivery of a digitizing system for X-ray and mammography devices, extensionan existing indirect digital radiography system used in SP ZOZ in Węgr-w and patient supervision equipment in the RTG Clinic

1. The subject of the order is the delivery of a digitizing system for X-ray and mammography devices, extension of the existing indirect digital radiography system used in SP ZOZ in Węgr-w and patient surveillance equipment in the periodRTG days

2. A detailed description of the subject of the order is contained in Annex 2 to the ToR, which is an integral part of it - 3. The minimum required deadline for granting the warranty on the subject of the order is 24 months counted from the date of protocol acceptance of the equipment by the Employer - the Contractor will meet the condition if he proves that he has own funds or creditworthiness in the previous periodthan 1 month before the submission of offers for the amount not less than 250, 000.00 PLN

1. The ordering party requires a bid bond in the amount of PLN 6, 776.00 (six thousand seven hundred seventy-six zlotys)

2. If the bid deposit is not completed by the deadline for submitting bids or incorrectly submitted, it will reject the offer of the contractor in accordance with Art. 89 paragraph 1 point 7b of the Public Procurement Law

3. Effective will bringthe tender security in cash takes place at the moment the cash is credited to the Contracting Authority's bank account, before the deadline for the submission of bids (ie before the day and the hour set as the deadline for submission of bids).

4. The content of the guarantee / surety should be unconditional, for every written request made by the Ordering Party within the binding period of the offer, the obligation of the Guarantorfor payment to the Ordering Party of the full amount of the bid bond in the circumstances specified in art. 46 ust. 4a and 5 Public Procurement Laws

5. The bid bond can be contributed in one or more of the following forms:

5.1. money transfer order to the bank account of the Employer: PEKAO SA Bank No. 69 1240 2731 1111 0010 1842 9333. with the note: "Wadium for the purchase of equipment of the computed tomography lab in the District Hospital in Węgr-w5.2. bank guarantees or credit and savings bond sureties, except that the surety is always a cash guarantee;

5.3 bank guarantees;

5.4 insurance guarantees,

5.5 guarantees provided by entities referred to in art. 6b paragraph 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (text from jedn. OJ from 2016, item 359, as amended.)

6. The bid security in a non-monetary form should be brought in the original in electronic form bearing a qualified signature. The submission of a security deposit in electronic form should include the transfer of this document in the form in which it was established by the guarantor, i.e. the original of the document. The content of the document should be unconditional fromoblige the Guarantor to pay the Ordering Party the full amount of the bid bond in the circumstances specified in art. 46 ust. 4a and art. 46 ust. 5 of the Public Procurement Law, for every written request made by the Employer at the time of being bound by the offer.

7. In the case of payment of a bid bond in the form of a guarantee or surety, it is necessary that the guarantee or guarantee covers liability for all cases causingce loss of the bid bond by the Contractor referred to in art. 46 ust. 4a and 5 Public Procurement Laws

8. The said guarantees and sureties may not introduce any additional substantive conditions.

9. The bid bond must cover the period of being bound by the offer referred to in Chapter IX of this Specification of Essential Terms of Orders.

10. The bid security brought by one of the consortium partners is considered to be broughtidłowo.

11. The Contracting Authority will refund the deposit pursuant to the rules set out in art. 46 of the Public Procurement Law
