Delivery and assembly of X-ray equipment for screening vehicles with associated functional and functional infrastructure at the international road border crossing in Bobrowniki

Subject of the orderis the delivery and assembly of a stationary X-ray device for scanning trucks, hereinafter referred to as "X-ray device" together with the associated functional and functional infrastructure at the international road border crossing point in Bobrowniki (16-040 Grodek).

1 The subject of the order is delivery and mona stationary X-ray machine for screening lorries, hereinafter referred to as "X-ray device" together with the associated functional and functional infrastructure at the area of the international road border crossing in Bobrowniki (16-040 Grodek).

2 The subject of the order includes in particular:

1) Preparation of technical opinion on functional and functional condition taken into accountem of its technical condition and suitability for further use of the existing X-ray device CAB 2000;

2) Development of detailed design documentation for the reconstruction of entry and organization of traffic and access in the newly installed X-ray installation area;

3) Expansion of the existing road system to the provision of an additional lane located outside the shelter with an X-ray device;

4) Disassembly istthe existing X-ray device CAB2000 on the site of the gmina in Bobrowniki together with folding it and moving it to the storage place indicated by the Employer at the site in Bobrowniki

Attention:

the Contractor will not be obliged to liquidate the dismantled X-ray device and transport outside dpg Boborniki!

5) Delivery and assembly of a new X-ray device for screening trucks, withnecessary software and licenses for installed computer programs along with a new service pavilion;

6) Installation of radiation shields, rebuilding of power and teletechnical connection;

7) Reconstruction of the shed (assembly from the north-east side of full curtain walls and partial curtain walls from the south side) west);

8) Employee training

9) Provision of serviceswarranty for an X-ray device for a minimum period of 7 years and for a period of 5 years for the works performed from the date of device reception in accordance with Annexes 1A-1D and the specimen agreement (Appendix No. 6)

List of declarations or documents confirming compliance with the conditions of participation in the procedure grounds for exclusion, and subject conditions:

1. The Contractor must submit via the POvalid as of the day of submitting bids, a statement in the form of a Single European Order Document (EAT). The information contained in the EED will constitute a preliminary confirmation that the Contractor is not subject to Exclusion and meets the conditions for participation in the procedure.

2. If the Contractor, refers to the resources of other entities in order to demonstrate compliance, in so far as it relies on their resources, un participate in the proceedings also submit a single document form regarding these entities. The form of a single document should be completed to the extent that the Contractor uses the resources of another entity.

3. The ordering party, before granting the order, will call the Contractor whose offer has been the highest rated, to submit within the prescribed period, not shorter than 10 days, currenton the day of submitting the following statements or documents:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

2) excerpt from the relevant register or from central records and business information, if separate regulations require entry into the registeror the register, in order to confirm the lack of grounds for exclusion under art. 24 sec. 5 point 1 of the Public Procurement Law Act,

3) the Contractor's statement on the lack of issuing a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or in the event of such a verdict or decision being confirmed by documentsmaking a payment of these receivables together with any interest or fines or entering into a binding agreement regarding the repayment of such receivables; - 4) a declaration by the Contractor about the lack of a ruling against him as a precautionary measure prohibiting to apply for public orders; - a list of deliveries made; periodic or continuous benefits also during the last period5 years prior to the deadline for submission of bids, and if the period of business is shorter - during this period, together with their value, subject, dates of completion and entities for which deliveries were made, and attaching evidence determining whether these deliveries were made or are duly performed, with the evidence referred to in the references or other documenty issued by the entity for which deliveries were made, and in the case of periodic or continuous services are performed, and if for a justified reason of objective nature, the Contractor is not able to obtain these documents - the Contractor's statement. In the case of periodic or continuous services, references still being made or other documents confirming their proper performanceshould be issued no earlier than 3 months before the deadline for submission of tenders - according to the template constituting Annex No. 5 to the ToR,

6) Functional and technical specification - Annex No. 1B to the ToR,

7) Declaration of the producer or equivalent document (issued by the manufacturer) or an authorized body in the country of the producer) confirming compliance with radiation safety standards by the offerorthe scanner defined in the Act of 29.11.2000 on Atomic Law (Journal of Laws of 2018 item 792), implementing regulations, and in European and international law regulations, including the International Atomic Energy Agency, the International Commission on Radiological Protection - ICPR60 and the World Health Organization;

8) CE Declaration of Conformity of the offered scanner or other legal documentny

4. Within 3 days of posting information on the Platform referred to in art. 86 par. 5 statement on belonging to or not belonging to the same capital group - the Ordering Party does not specify specific requirements regarding this condition.

Conditions for participation in the procedure and grounds for exclusion:

1. The Contractors, who:

1) can not be excluded, may apply for the orderfrom the proceedings

2) meet the conditions for participation in proceedings regarding: a) competence or entitlements to conduct a specific professional activity, if it results from separate regulations: Ordering does not specify specific requirements regarding this condition. b) technical or vocational capacity

The Contractor will meet this condition if he shows that in the last 5 years before the end of termnu tender, and if the period of business is shorter in this period, duly completed at least 1 contract, consisting of delivery with installation and commissioning of X-ray equipment to screen trucks or railway cars worth at least PLN 10, 000, 000.00 gross.

It is recommended to use the template constituting Annexe No. 5 to the ToR or presentany information required in the template with the value, subject, dates and subjects for which deliveries have been made, and the enclosure of evidence stating whether these deliverables have been duly carried out, the evidence referred to being references or other documents issued by the entity for which deliveries were made, and if justifiedreasons of an objective nature, the Contractor is not able to obtain these documents, the Contractor's statement.

c) economic and financial situation: the Employer does not specify specific requirements regarding this condition.

2. The Employer will exclude from the Contractor's proceedings:

1) who did not show compliance with the conditions of participation in the proceedings,

2) to whom the conditions specified in art. 24 par. 1 point 12-23 of the Public Procurement Law and paragraph 5 point 1 of the Public Procurement Law - For confirmation of the fulfillment of the requirements of the Contracting Authority, the following must be provided:

1) Functional and technical specification - Annex 1B to the ToR,

Completed in accordance with the requirements specified by the Employer. Functional and technical specification of the X-ray device constituting Annex 1B to SIWZ

2) Manufacturer's declaration or toEquivalent document (issued by the manufacturer or the authorized body in the producer country) confirming compliance with radiation safety standards by the scanner offered, specified in the Act of 29 November 2000 on Atomic Law (Journal of Laws of 2018, item 792), implementing regulations, and in European and international law, including the International Atomic Energy Agency, Miof the International Radiological Protection Commission - ICPR60 and the World Health Organization

3) Declaration of conformity of the offered scanner or other equivalent document
