sciascopic-sciagraphic set for examination and intervention in the field of GIT in accordance with the requirements set out in the technical specification, which forms Annex No. 4 to these ZP, its installation and commissioning, in the implementation of the necessary construction and othermodifications necessary for the assembly and installation of a new device, and minor construction modifications for the purpose of modernization of adjacent rooms not related to the installation of the set and the provision of subsequent service (warranty and post-warranty) X-ray set of the set under the conditions and to the extent defined in these ZP, incl. all attachments. The inspection of the place of performance will take place on 28 May 2020 at 2.30 pm

The subject of performance for the workplaces of radiodiagnostics and interventional radiology ZRIR consists of: - in the delivery of a new X-raysciascopic-sciagraphic set for examination and intervention in the field of GIT in accordance with the requirements set out in the technical specification, which forms Annex No. 4 to these ZP, its installation and commissioning, in the implementation of necessary construction and other modifications necessary for assembly and installation of a new device, and minor building modifications for the purpose of modernization of the adjacent affected rooms unrelated to the installation of the set and the provision of subsequent service (warranty and post-warranty) X-ray skiaset under the conditions and to the extent defined in these ZP, incl. all attachments. The inspection of the place of performance will take place on 28 May 2020 at 2.30 pm

§ 77 ZZVZ- paragraph 1, paragraph 2 letter a) -document of business license according to special legal regulations to the extent corresponding to the subject of the public contract:, their changes and removal or Finishing construction work; paragraph 2, letter c) -documents certifying the professional competence of the participant to handle the source of ionizing equipment to the extent necessaryto perform a public contract in accordance with the Atomic Act; a solemn declaration for the performance of instruction and service in the sense of Act No. 268/2014 Coll., on medical devices; a document certifying the professional competence of the supplier or a person through whose professional competence the supplier ensures the performance of electrical inspections in accordance with Section 67 of Act No. 268/2014 Coll.
