A detailed description of the subject of the order, including a description of the minimum technical and functional requirements is provided in Annex 4 to the ToR description of the subject of the order. demonstration of compliance by the Contractor with the conditions of participation in the proceedings and no grounds for exclusionthe entrant requires the following declarations and documents: a) a statement in the form of a Single European Order Document (EIN) - constituting Annex 2a to the Terms of Reference. In order to confirm the lack of grounds for exclusion of the contractor from participation in the proceedings, the Employer requires the following statements and documents: a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of offers; b) excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points 1 of the Act; c) declaration of the Contractor about the lack of release againstthe final judgment of the court or the final administrative decision on the late payment of taxes, fees or social security or health insurance premiums or in the event of issuing such a verdict or decision documents confirming the payment of such claims together with any interest or fines or a binding agreement regarding repayment of such payments accounts receivable;d) declaration of the Contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders; e) the Contractor's declaration of belonging or non-affiliation to the same capital group; in the case of belonging to the same capital group, the contractor may submit together with the declaration documents or information confirming that connections with anothercontractors do not lead to distortion of competition in the proceedings. Note: The Contractor submits the above document (letter e)) within 3 days from the date of placing by the Employer information on the opening of offers on the website (Article 86 (5) of the PPL Act). The information referred to in the statements of para. 2 lit. c and d The Contractor is obliged to submit in accordance with the specimen standard form EATspecified in the European Commission's implementing regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU in the part concerning the grounds for exclusion, while in relation to Chapter X, para. 2 letter e) in accordance with the guideline contained in Note3. In order to demonstrate by the Contractor the fulfillment of the requirements specified by the Ordering Party by the offered deliveries: The Ordering Party requires a description of the products whichto be delivered, i.e. technical and functional specification - ordered technical specification of the ordered item, allowing to verify any minimum requirements contained in the ToR and meeting each of the minimum parameters in accordance with the description of the subject of the order contained in the OPZ constituting Annex 4 to the ToR. Failure to meet the minimum requirements will resultrejection of the offer as inconsistent with the content of the ToR. The ordering party requires a description of products to be delivered in accordance with Annex No. 1a to the ToR.

In addition, the offer must be accompanied by the following documents: a) bid form in accordance with Annex No. 1, b) in the case of acting by a proxy, the offer must be accompanied by a power of attorney. The power of attorney must be submitted in the original forman item stamped with a qualified electronic signature or a notarized copy; c) an electronic document confirming the deposit payment
