1. The subject of the order is:

1.1. ensuring the provision of the Technical Support by the manufacturer of Servers and Software:

1.2. providing the Service Provider by the Contractor.

2. The detailed description and conditions and rules for the implementation of the order are specified in the Model Agreement constituting Annexe No. 6 to the ToR

In accordance with the provisions of art. 22 ust. 1 of the Act of 29.1.2004. Public Procurement Law (Journal of Laws of 2017, item 1579 with later amendments), hereinafter referred to as the Act on the award of contracts, may be applied to Contractors who:

1. they are not subject to the exclusion: a) on the basis of art. 24 sec. 1 of the Act;

b) on the basis of the circumstances referred to in Art. 24 sec. 5 points 1, 2, 4, 5, 6, 7 and 8 of the Act indicated in the Specifof the Essential Terms of Orders, hereinafter referred to as SIWZ, posted on the ordering party's website. The contractor is bound by the offer for a period of 60 days.

2. The ordering party sets a deposit for the entire subject of the order in the amount of 20, 000.00.00 PLN (in words: twenty thousand and 00/100). Detailed information on the bid bond is included in the ToR.

3. Criteria for the evaluation of offers and their meaningse: Gross price of the offer 100%;

4. The Contracting Authority does not provide for the ordering referred to in art. 67 sec. 1 point 6 of the Act

5. In the scope of the lack of grounds for exclusion, the Ordering Party shall demand according to art. 26 par. 1 of the Act and art. 24 sec. 11 of the Act of the following documents:

a) declarations and documents referred to in par. 5 points 1-9 of the Regulation on documents,

b) a statement about whichin par. 5 point 10 of the Regulation on documents, - c) documents and statements referred to in par. 7 and 8 of the Document Regulation and, where applicable, also par. 9 of the Document Regulation

6. The Contractor will submit the E-mail with the use of means of electronic communication via e-mail to the e-mail address: jedz@nbp.pl. Information on how to make up-to-dateo Uniform European Document, contains SIWZ.

7. The contracting authority provides for the application of the procedure to be followed, referred to in art. 24aa of the Act.

8. According to the content of art. 18 of the Act of 22.6.2016 on amending the Act on Public Procurement Law and some other acts (Journal of Laws of 2016, item 1020, as amended) proceedings were initiated before 18/10/2018 and the offers are made in the form roundeated in art. 18 point 4 of the Act of 22.6.2016
