3.1. The subject of the order is the delivery of liquid and enteral and parenteral feeding to the pharmacy closed to the place indicated by the Contracting Party. The detailed list of the product range is included in Appendix No. 2 attached to the Terms of Reference, code according to CPV: 33690000-3.

3.1. The subject of the order is the delivery of liquid and enteral and parenteral feeding to the pharmacy closed to the place indicated by the Contracting Party. The detailed list of the product range is included in Appendix No. 2 attached to the ToR, code according to CPV: 33690000-3.

3.2. The entire assortment should have certificates confirming admission to trading for thesePoland's rena

3.3. The order is divided into 16 sets (parts). It is allowed to submit partial offers for individual sets. As part of the kit, you should offer all of the listed items. Failure to complete any of the items in the set will result in rejection of the offer.

3.4. The subject matter of the order is to be new, admitted to trading on the basis of applicable laws and regulations.notify all requirements specified by law, in particular the Act of 6 September 2001 Pharmaceutical Pharmaceutical Law (Journal of Laws of 2017, item 2211, as amended), free from any physical or legal defects and have in on the day of delivery, expiry date, not shorter than 12 months.

3.5. The technical parameters given in the appendices should be treated as a minimum standardrequirements. 3.7. Equivalent product to product with the same chemical composition, which also means that the active substance must be in the same chemical compound, the same dose of the same medicinal product, with identical therapeutic indications, the same form and the same pharmacokinetic properties of

The declaration issued on the basis of art. 25a para. 1 and 2 of the Public Procurement Law in forme a single document of EAT according to a formula drawn up in accordance with the standard form specified in the Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for a single European order document (Official Journal of the European Union L. 2016.3.16 ) constituting - Annex 4 to the ToR.

In the case of submitting a partial offer for more than one set, the required deposit amount is appropriately summed up.

2. Contractors shall be excluded from the procurement procedure in accordance with Art. 24 sec. 1 and par. 5 points 1, 4 and 8 of the Public Procurement Law - 3. In addition, the offer should be accompanied by:

a. completed offer form (Annex No. 1 to the Terms of Reference).

b. completed form cenew (Annex No. 2 to the ToR). After selecting the best offer, the Contractors shall be obliged to send in an editable form the completed technical specification to the address l.grzesiuk@spzoz.powiatleczynski.pl. Due to lack of the above annexes, the offer will be rejected.
