- The subject of the public contract (VZ) is the delivery of a cystoscopic set for the examination of pediatric patients for the Urological Clinic of the General Faculty Hospital in Prague (VFN), including accessories, customs, transport, installation, commissioning, entry validation, instructionNo. 268/2014 Coll. on medical devices (hereinafter only Act No. 268/2014 Coll.), respectively. training the buyer and handing over the documents that are related to the delivered goods, especially the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including warranty service and other terms and conditions.The Public Contract (VZ) is a delivery of a cystoscopic set for the examination of pediatric patients for the Urological Clinic of the General Faculty Hospital in Prague (VFN), including accessories, customs, transport, installation, commissioning, entry validation, instruction according to the relevant provision of the Act. 268/2014 Coll. on medical devices (hereinafter only Act No. 268/2014 Coll.), respectively. trainedand the handling of the buyer and handing over the documents that are related to the delivered goods, in particular the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including the guarantee service and further according to other terms and conditions. it is possible to verify the entry in the Commercial Register at https://www.justice.cz/, if it is entered in it, or in the list, respectively. a direct link to anotheru similar records in which the supplier is registered according to special legal regulation (- 77 paragraph 1 zakona). The supplier encloses the affidavit that the registered information is up to date and truthful. The supplier may also demonstrate the eligibility of the document by submitting the document itself in electronic form;the scope corresponding to the subject of the public contract at http://www.rzp.cz/, in particular the document proving the respective trade license or license (Section 77 (2) (a) of the Act). The supplier encloses the affidavit that the registered information is up to date and truthful. The supplier may also demonstrate the eligibility of the document itself in electronic form;
