3.1 The subject of the order is the supply of fuel: unleaded PB 95 petrol and diesel oil to tanks at the service station of the Transport Department of the Kielce Regional Headquarters in Kielce.

Main Site: The subject of the orders should be delivered in batches in accordance withwith the ordering of the Ordering Party to the warehouse of the Transport Department of the Provincial Police Headquarters in Kielce, ul. Kusocińskiego 51

3.1 The subject of the orders is the supply of fuel: unleaded petrol PB 95 and diesel fuel to tanks at the filling station of the Transport Department of the Kielce Military Works.

3.2. .

3.3 Providedproducts should have valid product safety data sheets in accordance with Regulation (EU) No. 453/2010 and classification in accordance with Directive 1999/45 / EC. • The license for liquid fuels issued on the basis of art. 32 para. 1 Energy law.

Confirmation must be submitted by documents proving that the Contractor is insured against liability-in the scope of conducted activities related to the subject of the order for a guarantee sum not less than PLN 3, 000, 000.00 gross. The Contractor in a uniform document is obliged to indicate the amount of insurance, while on the Employer's request he will submit a document confirming insurance. In order to demonstrate the lack of grounds to exclude from the procurement procedure, awca will submit a statement on the lack of grounds for exclusion (uniform document) and submit to the ordering party's request the following documents and statements:

a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submitting offers;

b) the certificate of the competent head of the tax officethat the Contractor is not in arrears with the payment of taxes, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of such receivables with possible interest or fines, in particular the exemption, deferral or distribution provided for by lawoverdue payments or suspension in full of the decision of the competent authority; - c) certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued no earlier than 3 months before the end of the daym the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of such receivables, including possible interest or fines, in particular obtained legally permissible, postponement or installment payment of overdue payments or suspension in full execution of the decision of the competent (d) a copy from the relevant register or from-Central Register and Information on Business, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Article. 24 sec. 5 point 1 of the Act;

e) declaration of the Contractor about the lack of issuing a final court judgment or final administrative decision on default of payment of taxes, fees or contributions forsocial or health insurance or in the event of issuing such a verdict or decision documents confirming the payment of these receivables with possible interest or fines or conclusion of a binding agreement on repayment of such receivables; f) declaration of the Contractor about the lack of a ruling against him as a preventive measure for public orders

g) Contractor's statement on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716).

h) a statement that The contractor has not been the subject of a final conviction of a conviction court for a restriction of liberty or a fine to the extent specified by the awarding entity on the basis of art. 24 sec. 5 pkt 5 Pzp

i) a statement that in relation to the persons referred to in art. 24 sec. 5 point 6 of the PPL, no verdict of the court convicting for offense against restriction of liberty or a fine to the extent specified by the Awarding Entity under Art. 24 sec. 5 point 6 Pzp;

j) a statement that in relation to the Contractor, which he represents, no final administrative decision was made about breach of obligationsprovisions of labor law, environment protection law or provisions on social security in the scope determined by the contracting party on the basis of art. 24 sec. 5 point 7 Pzp;

If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to above, he shall submit the documents specified in the Regulation of the Minister of Development of 26.7.2016 intypes of documents that contractors may request from the contractor, their validity period and the forms in which these documents may be submitted (Journal of Laws of 2016, item 1126).
