The subject of the order is the delivery of equipment described in Annex 1.1 to the ToR, i.e. ultrasound apparatusas part of the project "Increasing the quality and availability of medical services of the SPWSZ in Szczecin by building a building for the needs of the Department of Nephrology and Kidney Transplantation, Dialysis Station, the Department of Neurology together with the Stroke Department and the Department of Internal Diseases and Hypertension with equipment" for the SPWSZ in Szczecin, implemented as part of a project co-financed by the Polish originfrom the budget of the European Union, as part of the regional operational plan of the West Pomeranian Voivodship entitled "Increasing the quality and availability of medical services of SPWSZ in Szczecin by building a building for the needs of the Department of Nephrology and Kidney Transplantation, Dialysis Station, Department of Neurology together with the Stroke Department and the Department of Internal Diseases and Hypertension with accessorieseń ".

Performance of the subject of the contract is co-financed from funds European Regional Development Fund and on the basis of funds from the state budget on the basisi.e., the contract for co-financing of the project `Improving the quality and availability of SPWSZ medical services in Szczecin concluded on 7.12.2017 by building a building for the Department of Nephrology and Kidney Transplantation, Dialysis Station, Department of Neurology together with Stroke Department and Department of Internal Diseases and Hypertension with equipment "No. RPZP.09.01.00-32-0003 / 17-00 as part ofof the West Pomeranian Voivodship Operational Program for 2014-2020, Priority Axis 9: "Public Infrastructure", Measure 9.1 "Health Infrastructure" (hereinafter "Subsidy Contract").

1 Communication between the Employer and Contractors takes place using the SmartPZP Platform (hereinafter the "Platform").

2. In order to use the Platform, it is necessary to register the Contractor at: https://portal.smartpzp.pl/spwsz after clicking the "Create Account" button.

3. Each Contractor is required to attach to the offer (prepared according to the template, see Appendix 2 to the ToR for the tasks on which the offer is submitted):

1) a statement to confirm compliance with the conditions for participation in the procedure and the lack ofContractor`s exclusion terms. Declaration. in accordance with art. 25a section 2 of the Public Procurement Law, the Contractor shall submit in the form of a single European order document (EAT), in accordance with the specimen Annex No. 3 to the ToR;

2) technical specification description of the object of the contract (required parameters), according to the model Annex No. 1.1 to the Terms of Reference;

3) technical parameters subject to qualitative assessment, according to the model Annex No. 1A.1 toToR;

4) obligations of third parties, deaths with pattern state Annex No. 5 to the ToR, together with documents (e.g. excerpts from the register, powers of attorney) confirming the authorization of the person / persons who signed the commitment to represent the third entity in the event of the Contractor referring to the resources of other entities to demonstrate compliance. conditions for participating in the proceedings;

5) fully applicablepowers or documents (e.g. excerpts from the register) showing the authorization of the person signing the offer to represent the Contractor (if applicable);

6) documents describing the equipment offered by the Contractor (catalog pages or folders, information leaflets, etc.) in Polish for the offered devices allowing confirmation of their compliance with the Terms of Reference regarding the requirements specified by the Employerego in Annex No. 1.1 to the ToR and to the extent of the Contractor`s statements as to compliance. parameters subject to assessment in the scope of quality criteria contained in Annex No. 1A.1 to the ToR.

4. The Contractor is subject to exclusion if any of the conditions specified in art. 24 paragraph1 point 12 23 Public Procurement Law or art. 24 paragraph 5 points 1, 3 and 8 of the PPL Act

In order to confirm satisfy. conditions for participation in the procedure and the absence of grounds for excluding the Contractor from participation in the procedure, the Employer before awarding the contract, will call the Contractor whose bid has been best rated to be submitted within a designated period, not shorter than 10 days, valid on the date of submission next. declaration. and documents:

1) information from the NQF in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law;

2) a certificate from the competent head of the Tax Office confirming that the Contractor is not in arrears with tax payments;

3) a certificate from the appropriate local organizational unit of ZUS or KRUS or another document confirming that the Contractor is not in arrears payment of social security or health insurance contributions;

4) copy from the properof the register or from the central register and information on economic activity, if separate provisions require an entry in the register or the register, in order to be confirmed. no grounds for exclusion under Article 24 paragraph 5 point 1 of the Public Procurement Law;

7) in order to demonstrate compliance with the condition of participation in the proceedings regarding technical or professional capacity referred to in Chapter V point 1 point 2 lit. c Terms of Reference:
