Cutting, planting and care of trees along voivodship roads administered by the Voivodship Road Administration in Zielona G-ra. Voivodship Road Region in Zielona G-ra

a) Mech. tree felling with trunk milling.

a) Mechanical shearinge trees with milling trunks to a depth of min. 10 cm below the nominal height of the sidewalk or area along with the clearing of the area and the export of logs, branches.

B) Mechanical cutting of trees along with the clearing of the area and the removal of logs, branches (without milling, e.g. in the case of tipping, where it will also be necessary to grub up trunks).

c) Removal of (dense) bushes along with the removal of logging residuesand tidying up the area.

d) Rejuvenation of older trees, including tree crown cutting (up to 30% of the tree crown) and treatments to obtain a vertical road gauge.

e) Removal of broken, dead branches from trees using a hoist.

f) Cutting branches and regrowths growing into the light of the road gauge.

g) Mechanical grubbing up of trunks along with covering the root gap, compactedand disposal of grubbing up residues for disposal.

h) Milling trunks to a depth of min. 10 cm below the nominal height of the pavement or area.

I) Planting deciduous trees with tanning (3 stakes) and seedling care; seedling height 2 2.5 m; trees with trunk circumference min. 10 cm, common, ornamental species such as small-leaved linden, Norway maple, common rowan, horse chestnut and tp.

j) Planting deciduous trees with tanning (3 stakes) and seedling care; seedling height 2 2.5 m; trees with trunk circumference min. 14 cm, common, ornamental species such as small-leaved lime, Norway maple, common rowan, horse chestnut, etc.

k) Planting deciduous trees with tanning (3 stakes) and seedling care; seedling height 2 2.5 m; trees with trunk circumference min. 10 cm, spherical and columnar species, such as "Globosum" maple, "Fastigiata" oak, etc.

l) Filling of the stakes with tape (set: 3 stakes with a minimum diameter of 5 cm, webbing tape, width 4 cm min.) .

Ł) Clearing weeds and grass of the green roundabout along with trimming the existing shrubs.

The services will be provided by the contractor at various times depending on the needs:

1) in emergency situationsdemanding road safety, the maximum time for commencing implementation and securing the area will be ... (in accordance with the contractor`s offer) from the moment of accepting the order by fax / telephone *, prior to commencing the implementation, temporary marking and securing of the workplace should be performed;

2) in other situations, the time will be determined each timeimplementation from the moment of accepting the order, by fax or e-mail.

* telephone orders will apply only on non-working days and public holidays and outside RDW Zielona Gora office hours; Telephone orders will be confirmed in writing the next business day.

A detailed description of the subject of the contract and technical requirements are set out in Chapter III SIWZ.

The contractor will meet the condition if he demonstrates a minimum level of capacity, i.e. jointly operating entities, the condition may be met jointly.

At the request of the contracting authority, the contractor is obligedy is to submit the following statements or documents:

1) documents confirming that the contractor is insured against civil liability in the scope of conducted activities related to the subject of the contract for a guarantee sum not less than: PLN 90, 000.00.

If for a justified reason the contractor cannot submit the documents required by the contracting authority, referred to above, he ordersthe contractor allows the contractor to submit other documents referred to in art. 26 section 2 c of the Public Procurement Law

I. The contracting authority envisages the use of the procedure referred to in art. 24 aa paragraph 1 of the Public Procurement Law

II.1. The contractor is obliged to attach to the offer a statement valid as at the date of submission of tenders constituting a preliminary confirmation that the contractor:

a) is not subject to exclusion;

b) meets the conditions for participationin the proceedings.

2. The contractor referred to in item 1 is obliged to submit in the form of a single European order document (EAT) prepared in accordance with the template of the standard form set out in the European Commission implementing regulation issued on the basis of art. 59 clause 2 of Directive 2014/24 / EU, hereinafter referred to as the "single document" (Annex 1 to the formoffers) under pain of nullity, in electronic form, bearing a qualified electronic signature in accordance with the principles of point 8.2 of the ToR. The EAT document should be attached to the offer, and then, together with the files constituting the offer, be compressed into one archive file (ZIP).

3. The contracting authority requires the contractor to fulfill only section a in terms of the qualification criteria described in Part IVlfa in part IV and did not complete any of the other sections in Part IV.

III. Prerequisites for the exclusion of performers:

1. The contractor, in relation to which any of the circumstances referred to in art. 24 paragraph 1 point 12 23 of the PPL Act.

2. In addition, the contracting authority will exclude the contractor based on Art. 24 paragraph 5 points 1, 5, 6, 7, 8 of the PPL Act.

IV. Onthe contracting authority`s request the contractor is required to submit the following statements or documents: In order to confirm the lack of grounds for excluding the contractor from participation in the procedure, the contractor is obliged to submit RMR compliant documents from 26.7.2016 regarding the types of documents that the contracting authority may request from the contractor in the procurement procedure ( Journal of Laws of 2016, item 1126), i.e. 5 points:1), 2), 3), 4), 5), 6), 7), 8), 9). If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, and also if the person to whom the given document relates has his place of residence outside the territory of the Republic of Poland, paragraph 7 above applies. V. The contractor is bound by the offer 60 days from the submission deadline. The offer validity period begins with the submission deadline. (Article 85 (5) of the PPL Act).

VI. Deposit:

1. The contractor is obliged topaying a tender bond in the amount of PLN 4, 000.00 before the bid submission deadline.

3. The bid bond in the form of money must be transferred to a bank account at Getin Noble Banks.AO/Zielona Gora account number 38 1560 0013 2263 6869 1000 0011 with a note on the transfer: "Bid bond in proceedings ZDW-ZG-WZA-3310-23 / 2020 on Felling, planting and care of trees on voivodship roads administered byBoard of Voivodship Roads in Zielona G-ra Region of Voivodship Roads in Zielona G-ra.

VII. Along with the offer, the contractor is obliged to submit via the miniPortal:

1) EAT;

2) the obligation to provide resources with Annex No. 2 to the offer form (if the contractor relies on the resources of another entity;

3) completed price form;

4) original warranty or surety if the bid bond is paidis in a form other than money, subject to the provisions of point 12 of the Instruction;

5) power of attorney lodged in the original, in the form of an electronic document with a qualified electronic signature or in an electronic copy certified as true to the original, and provided with a qualified electronic signature by a notary public.
