Conclusion of a framework agreement for the supply of consumables for printing devices, in the form toner and photoconductor. A detailed description of the subject of the order is specified in Annex No. 1.1 to the ToR for part no1 and Annex No. 1.2 to SIWZ to Part No. 2.

The subject of the order is the supply of toner to printing devices. A detailed description of the subject of the order, i.e. quantity and type, is specified in Annex 1.1 to the ToR.

Contractors fulfilling the conditions for participation in the procedure may apply for the award of a public contract and who are not subject to exclusion under Art. 24 sec. 1 point 12-23and 24 sec. 5 of the Act

1. The orderer requires a deposit to be paid to part No. 1 - PLN 40, 000.00, to parts No. PLN 2, 900.00. The rules regarding the deposit are set out in point VIIII of the ToR.

2. The grounds for exclusion and documents submitted in order to confirm the absence of grounds for exclusion have been set out in point VI. TOR

3. The offer and documents submitted together with the offer - are specified in point IV of the ToR.

4. The orderer will use pthe procedure referred to in art. 24aa of Public Procurement Law

5. Document confirming that the offered deliveries meet the requirements of the Ordering Party described in Annex No. 1.1 to the SETC - are set out in point VII of the ToR - at the request of the Employer.

6. According to art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of natural persons in connection with the processing ofpersonal data on the free movement of such data and the repeal of Directive 95/46 / EC (general regulation on data protection) (Official Journal L 119, 4/4/2016, page 1), hereinafter "RODO", I inform that:

- your personal data will be processed on the basis of art. 6 par. 1 lit. c ROPE for the purpose related to the public procurement procedure number ZZP-2380-47 / 2019, conducted as an open tender, - the recipients of your personal data will be persons or entities to whomdocumentation of proceedings based on art. 8 and art. 96 para. 3 of the Act of 29.1.2004, PPL (Journal of Laws of 2013, item 1986 as amended), hereinafter the "Pzp Act", - your personal data will be stored in accordance with Article 97 paragraph 1 Public Procurement Law, for a period of 4 years from the date of completion of the procurement procedure, and if the duration of the contract exceeds 4 years, the retention period fortakes the entire duration of the contract,

- the obligation to provide your personal data directly to you is a statutory requirement set out in the provisions of the Public Procurement Law, related to participation in the public procurement procedure; the consequences of failure to provide specified data result from the Public Procurement Law, - in the case of your personal data decisions will not be taken in a wayexpanded, applied to art. 22 RODO,
