Acquisition of CT Scanner

wishes to acquire a CT scan, as required by the Task Force, appendix to this.

1) Requirement:

Tenderers participating in the procedure will declare that they do not fall within the situations provided by art. 59, art. 164, 165 and 167 of Law no. 98/2016 on public procurement

1) Mode of accomplishment:

At the time of submitting the offer, the participating economic operators will have to declare / confirm by DUAE their non-compliance with the reasons for exclusion from the urman procedured as reported to the provisions of art. 196 of Law no. 98/2016 the contracting authority shall require the tenderer ranked first in the intermediate hierarchy following the completion of the evaluation of the tenders to submit: - the criminal record certificate of the economic operator and of the members of the management, management or supervisory body of the economic operator have power of representation, decision or control within it as it results from the certifying certificate issued by ORC / constitutive document or, as the case may be, documents proving that the economic operator may benefit from the derogations provided in art. 166 par. (2), art. 167 par. (2), art. 171 of Law no. 98/2016 on public procurement, - documents issued by competent legal authorities, valid at the time of their presentation, stating that the tenderer has no overdue debts at the moment of their presentation, ie that he has fulfilled his obligations to pay the taxes and duties to the consolidated general budget, as well as the local taxes (or equivalent acts for the foreign operators ), as well as the criminal record of the bidding company and of the legal representatives aNote 2: If the third party / third party, the subcontractor / subcontractor falls within one of the grounds for exclusion provided in art. 59, 164, 165 and 167, the contracting authority shall request, once only, that the economic operator to replace the third party / supporter / supporter, reson the subcontractor / subcontractors; 2) Requirement:

1) Requirement:

Othe economic contractors submitting a bid must prove a form of registration under the law of the country of residence, submit documents showing that they are legally constituted, that they are not in any of the situations of cancellation of the establishment and that they have the professional capacity to perform the activities that are the object of the contract.

Specify the annual average turnover. The bidders will prove that over the past 3 years they have registered a global average turnover that is at least double the estimated value of the procedure, namely RON 2, 823, 529.40.

For viewing the awarding documentation uploaded in SEAP, economic operators must have a required program to view the signed filese (electronic signature providers` websites) according to art. 60 par. (4) of GD 395/2016.

Rules for communication and data transmission:

- requests for clarification regarding this tender documentation will address exclusively in the SEAP in the "Questions" section of the award procedure conducted by electronic means, and the answers to them willto be published in the SEAP both the "Questions" section and the "Documentation, clarifications and decisions" section of the contract notice, the contracting authority not to respond to the requests addressed by another means of communication than the one established in accordance with the provisions of Art. 64 par. (1) of the Law no. 98/2016 on Public Procurement,

- for requesting transmissionthe economic operators will register with SEAP (www.elicitatie.ro) as an economic operator and as a participant in the award procedure, - for the subsequent posting of tenders: The evaluation committee will send the requests for clarification in the connection with the offer by using the technical facilities available in SEAP (section "Intr- the economic operators will send the answers to the clarifications and any documents requested during the evaluation of the tenders through the SEAP.

The contracting authority reserves the right that if the source of financing does not materialize to order the cancellation of the award procedure, the decision to be adopted at the latest on the date of the report to be attributedre.
