Statement by the OE, signed by the legal representativepro tempore, using models DGUE, with which the declarant certifies:

a) that he is not in any of the exclusion conditions provided for by art. 80 of Legislative Decree 50/2016 and subsequent amendments (Part III of DGUE);

b) that the company is in compliance with the rules governing the right to work of the disabled, pursuant to art. 17 of L.68 / 99; companies that employ less than 15 employees will have to declare this situationaction; must also be indicated by the provincial office responsible for issuing the certification to issue the certification attesting the aforementioned declaration (part III D of DGUE);

c) to be registered with the C.C.I.A.A., ex L. 15.5.1997 and n. 127 and D.P.R. 28.12.2000 n. 445, providing all the information contained in the certificate of registration, (part IV of DGUE) and indicating theTerritorial Chamber of Commerce;

d) the requirement of the absence of mafia or camorristic procedures against the legal representative of the company and, where present, of the subjects referred to in art. 80, c. 3, of Legislative Decree 50/2016 (Part III, Section D, of DGUE)
