Supply and installation n. 1 TC 128 slices for the shoedale di Foligno.

The auction base relating to lot n. 1 and 615 328, 16 EUR over VAT and over the costs of security for interference risks amounting to 500.00 EUR.

Declaration by the Economic Operator, digitally signed by the Legal Representative, via model DGUE, which is available in editable version on the website http://www.uslumbria2.it/amministrazione-trasparente/bandi-di-gara-e-contracts.

The D.G.U.E. must be provided in electronic format, on a USB stick and attached to the application for participation, which states:

a) not to be in any of the exclusion conditions provided for by art. 80 of Legislative Decree 50/2016 and subsequent amendments. (Part III of DGUE);

b) that the company is in compliance with the rules governing the right to work of the disabled, pursuant to art. 17 of the-Law 68/99.

Companies employing fewer than 15 employees must declare this situation; the Provincial Office responsible for issuing the certification attesting to the aforementioned declaration must also be indicated (Part III, Section D of the DGUE); c) to be registered with the C.C.I.A.A., ex L. 15.5.1997 n. 127 and D.P.R. 28.12.2000 n. 445, providing all the information contained in the certificateinscription, (within the space, Part IV, section A, point 1 and Part II, section A, of DGUE) and indicating the territorially competent Chamber of Commerce; d) the requirement of the absence of mafia proceedings or camorristici to load of the legal representative of the firm or of the directors or partners furnished of rappresentanza, or unbounded (Section III, section D of the DGUE).Suitable bank references, attached in original, issued by at least 2 (two) credit institutions certifying the financial and economic capacity of the competitor adequate for the execution of the contract and that the competitor is able to meet regularly and punctually commitments. The economic operator must declare that encloses no. 2 bank references using the model D.G.U.E. precisely in part IV of the section, point 6).

If the company can not present the 2 bank references, it will have to specify the reasons, in part IV of the section, point 6) of the DGUE, indicating, in part IV of the section to the point 1a), the annual turnover () of the company in the three-year period 2015 2016 2017. If the economic operator does not have the above information, -relating to the three-year period 2015 2016 2017, must also complete part IV of the section, point 3) of DGUE.

The present call for tenders has been issued by decision of the General Director of the Local Health Unit of Umbria 2 n. 1447 of 8.10.2018.

Pursuant to art. 76 Legislative Decree 50/2016 and subsequent amendments, in Part II, section of the DGUE, for the purpose of sending communications, it must be obligedthe domicile elected and the certified e-mail address indicated.

The DGUE must be completed only in the parts not expressly crossed out and accompanied by the documentation referred to in this notice.

Specifically and temporarily grouped companies and consortiums are allowed to bid. pursuant to art. 45, 47 and 48 of Legislative Decree 18.4.2016, n. 50 and ss.mm.ii ..

Inin the event that the application for participation is submitted by enterprises under a temporary grouping scheme, the DGUE and the documentation referred to above must be submitted by each economic operator participating in the grouping.

The amount relating to each individual lot and based d 'auction and at the time of the offer can not be exceeded under penalty of exclusion from the race.

The modalities of presentationIn any case, the offers will be indicated in the letter of invitation and the technical specifications will be described in detail in the special tender specifications (this documentation will be sent to the companies admitted, after the pre-qualification phase). Pursuant to art. 105, paragraph 6, of Legislative Decree No. 50/2016 and subsequent amendments, in the case of subcontracting, for competitors it is mandatory to indicate, at the time-of the offer, the trio of subcontractors. The application for participation does not bind the Administration which reserves the right to revoke, annul, in whole or in part and at any time, this procedure, for reasons of convenience or public interest, without the competitors being able to make claims or rights of any kind.

The Local Health Unit of Umbria 2 reserves the right toto be awarded even if only one bid is received or only one offer is still valid, provided that it is considered appropriate at the sole discretion of the same Local Health Unit Umbria 2. The award will be made for each lot.

The award criteria are as follows:

- that of the most economically advantageous tender, based on the-parameters that will be indicated in the invitation letter for lot n. 1 and the lot n. 2

- that of the lowest price for lot n. 3

The envelope containing the DGUE and the above documentation must reach the Local Health Unit of Umbria 2 Centro Direzionale Via L.

The changes to the contract-they can be made according to the provisions of the art. 106 of Legislative Decree no. 50/2016 and subsequent amendments.
