5.1 The subject of the order is delivery of professional electroacoustic equipment, audio-video, internal communication system (intercom) and media server, according to the following statement:

1. Notes to Section IV.2.6 .: The offer binding period is 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires.

2. The Contractor shall be excluded from the procurement procedure in relation to which any of the circumstances referred to in art. 24 sec. 1 point 12-23 of the Public Procurement Law - 3. In addition, the Employer will exclude the Contractor:

a) in relation to which the liquidation was opened, in the system approved by the court in the restructuring proceedings, it is foreseen that the creditors will be satisfied by the liquidation of their property or the court ordered the liquidation of its assets pursuant to art. 332 par. 1 of the Act of 15.5.2015. Restructuring law (Journal of Laws of 2015, item 978, 1259, 1513, 1830 and 1844) or whose bankruptcy was announced, except for the Contractor who, after the announcement of bankruptcyand concluded a system approved by a valid court order, if the arrangement does not provide for the satisfaction of the creditors by liquidating the assets of the bankrupt, unless the court ordered the liquidation of its assets in accordance with art. 366 para. 1 of the Act of 28.2.2003 Bankruptcy Law (Journal of Laws of 2015, item 233, 978, 1166, 1259 and 1844 and of 2016 item 615),

b) who breached the obligations regarding payment of taxesow, fees or social security or health insurance premiums, which the Purchaser is able to demonstrate by means of appropriate evidence, except in the case referred to in art. 24 sec. 1 point 15 of the Public Procurement Law, unless the Contractor has made payments of taxes, fees or social or health insurance contributions, including interest or fines, or has entered into a binding agreement in the caseknows the repayment of these receivables.

4. The Contractor may, in order to confirm compliance with the conditions of participation in the procedure, in appropriate situations and with regard to the order or parts thereof, rely on the technical or professional abilities of other entities, regardless of the legal nature of the legal relationship he has with it. Contractor who relies on the abilities or situation of other entities, -he must prove to the Employer that while performing the order, he will have the necessary resources of these entities, in particular by presenting the obligation of these entities to give him the necessary resources for the order's execution

4. In order to confirm the lack of grounds for excluding the Contractor from participation in the proceedings, the Contracting Authority requires: a) information from the National Court Registerego [...],

b) certificate of the competent head of the tax office [...],

c) certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund [...],

d) excerpt from the relevant register or from the Central Register and Information on Economic Activity, - e) statements of the Contractor about the lack of a final court judgment against himu or a final administrative decision on defaulting on payment of taxes, fees or social security or health insurance contributions,

f) the Contractor's statement about the lack of a decision against him as a preventive measure of the prohibition on the application of public orders; The ordering party demands a deposit (in the amount separate for each part of the order) and security of due performancecontract (in the amount of 2%).

6. In the proceedings, the declaration shall be made in writing or in electronic form, except that the single document (EAT) shall be sent in electronic form bearing a qualified electronic signature. The Employer is the administrator of personal data in the present proceedings regarding physical persons. Detailed information about the processing of personal datathey are in the SIWZ.
