The subject of the order is the supply of telephone sets, attachments and telephone parts for institutions in the supply of the Cyberspace Resource Center of the Armed Forcesother, divided into 4 parts:

The subject of the order is delivery:

1. ATS-2D special telephone set with electrohermetic enclosure;

2. industrial telephone set without a keypad;

3. secretary-director`s telephone;

4. single-line telephone set with display;

5. telephone setlinear with display;

6. single-line telephone set with display;

7. single-line telephone set with display;

8. single-line telephone set without display;

9. system telephone set;

10. system telephone set;

11. single-line telephone set without display;

12. industrial telephone set.

Purchaser nospecifies in this respect any requirements which the Contractor is obliged to demonstrate in a special way. 24aa of the PPL Act. This means that the Purchaser will first evaluate the offers and then examine whetherthe bidder whose offer has been initially assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings. Contractors who are not subject to exclusion, as referred to in art. 24 paragraph 1 point 12 23 of the Public Procurement Law and art. 24 paragraph 5 points 1, 4 and 8 of the PPL Act. Contractor in accordance with art. 24 paragraph 11 of the Public Procurement Law, within 3 days from the date of the changeinformation on the website referred to in art. 86 section 5, provides the Employer with a declaration of belonging or not belonging to the same capital group (template Annex 7 to the ToR). The Contractor may submit along with the statement documents or information confirming that links with another Contractor do not lead to distortion of competition.

To confirmconditions for participation in the proceedings, the Employer will request:

a) a list of deliveries made, and in the case of periodic or continuous services also performed, within the last 3 years before the date of submission of tenders, and if the period of activity is shorter in this period, together with the subject, dates of execution, gross value and entities for which deliveries were made and fromattached evidence whether they have been properly performed (Annex 10 to the ToR);

In order to demonstrate the lack of grounds for exclusion from the contract award procedure in the circumstances referred to in point 6.1. ToR, the Employer will request the submission of the following documents:

a) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act, issuedj not earlier than 6 months before the deadline for submission of bids;

b) an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or the register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;

c) the Contractor`s statement on the lack of issuing a valid product against himtowards a court or final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the event of such a judgment or decision on documents confirming the payment of these claims together with any interest or fines or the conclusion of a binding agreement on the repayment of these claims Annex 5 to the SIWZ;

d) the Contractor`s statement on the lack of a decision against him as a preventive measure prohibiting him from applying for public contracts model Annex 4 to the ToR;

d) the certificate of the competent head of the tax office confirming that the Contractor is not in arrears with tax payment, issued not earlier than 3 months before the deadline for submission of bids;

e) a certificate from the competent authorityan organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund (Kasa Rolniczego Ubezpieczenia Społecznego) or another document confirming that the Contractor is not in arrears with the payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders. Parlof European and Council Regulation (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (hereinafter the "GDPR"). Contractor the person submitting the offer in the procedure is obliged to submit the statement required from the Contractor in the scope of fulfilling his information obligations.
