1.In the present proceedings, the procedure set out in Article 24aa of the Public Procurement Law will apply. 2. Every contractor will be bound by his offer 60 days after the deadline for submitting bids.

3. The contractors are obliged to submit the following documents and statements:

3.1. Offer structure - in the case of submitting an offer by jointly appearing entities, the names (companies) andfull addresses of all contractors submitting a joint offer.

3.2. Certification about the part of the contract, the contractor intends to entrust subcontractors and subcontractors - in the case of contracting subcontractors by contractors.

3.3. Additional scoring parameters and a list of devices offered. Representation to represent in the proceedings or repreview in the procedure and conclusion of the contract, in the case of contractors jointly applying for the award of the contract in accordance with art. 23 of the Act on Public Procurement Law (also applies to civil partnership partners).

3.5. Power of attorney to act on behalf of the contractor, if the documents making up the application are signed by a person whose authorization to represent the contractor will not beresult from the relevant register or from the central register and information about business activity. 4. The purchaser will exclude from the proceedings the contractor subject to exclusion pursuant to art. 24 sec. 1 of the Act.

5. List of declarations or documents submitted by the contractor for the purpose of preliminary confirmation that it is not subject to exclusion and meets the conditions for participation in the proceedings and meets-selection criteria:

5.1. Single European order document - the contracting authority recommends the contractors to read the instructions for completing the document on the website of the Public Procurement Office at: https://www.uzp.gov.pl/__data/assets/pdf_file/0015/32415 /Instrrukcja-wypelniania-JEDZ-ESPD.pdf.

5.2.Wiekonawca, which is subject to exclusion under Art. 24 sec. 1 point 13 and 14 and 16-20, pote provide evidence that the measures taken by him are sufficient to demonstrate its reliability, in particular to prove compensation for damage caused by an offense or fiscal offense, financial compensation for harm suffered or remedied, an exhaustive explanation of the facts and cooperation with law enforcement authorities and taking specific technical meansh, organizational and personnel, which are suitable for preventing further offenses or fiscal offenses or improper conduct of the contractor. The provision of the first sentence shall not apply if the contractor, being a collective entity, has been the subject of a final court decision prohibiting to apply for the order and the period of validity specified in that judgment has not expired.5.3. The contractor, which relies on the abilities or situation of other entities, must prove to the orderer that while fulfilling the order, he will have the necessary resources of these entities, in particular presenting the obligation of these entities to give him the necessary resources for the implementation of the order . In order to assess whether the contractor relying on the abilities or-the situation of other entities under the terms set out in Article 22a of the Act, will have the necessary resources to the extent enabling the proper implementation of the public order and assess whether the relationship between the contractor and these entities guarantees real access to their resources, document must specify in particular: a) the scope of the contractor's resources of another entity; b) use itresources of another entity, by the contractor, when performing the public order; c) scope and period of participation of another entity in the performance of the public order; - Continued in point VI.4.3).
