The subject of the order is the delivery of equipment for the workshophi didactic aids to the Special Schools Complex in Śrem, divided into 8 following parts: Part no. 1 outfit of school rooms Part no. 2 outfit of a natural laboratory Part no. 3 outfit of school rooms in books Part no. 4 outfit of school rooms Part no. 5 outfit of a natural laboratory and Mathematics Part no. 6 - equipment for school-class studios C# 7 - sports equipment Part No. 8 - technical and auxiliary equipment 2. The detailed scope of the subject of the order and the list of material, quantitative teaching aids and equipment of the workshop are provided in Annex 7.1, 7.2, 7.3, 7.4, 7.5, 7.6, 7.7, 7.8 and Annex No. 6.1. and 6.2. to SIWZ. 3. As part of part no. 1, equipment for school rooms should be provided. The detailed scope of the subject of the order and the physical, quantitative list of didactic aids and the equipment of the workshop are provided in Annex 7.1 and 6.1 to the ToR. Aidsdidactic and equipment of the subject laboratories should have appropriate certificates, quality certificates and meet all the requirements of standards defined by applicable law. The delivered order items must be brand new, free of defects, packaged in packaging that allows unambiguous identification of the product. It is recommended that the delivered assortment be packed in the cardboardnew boxes that prevent damage to products during transport. Responsibility for damage to products during transport is borne by the Contractor. If in the description of the subject of the order the names of devices of specific producers, trademarks, patents, origin, standards, approvals, technical specifications and technical reference systems are given, then this should be treated onlyas well as defining the desired standard and quality. In all such situations, the Contractor may offer equivalent products or devices with at least the same parameters. By equivalence of the product is meant to offer a product whose technical parameters of the materials used are at least the same as those described in the ToR. The Awarding Entity does not provide for the award of contracts referred to in art. 67 sec. 1-points 6 and 7 of the Act. The period of two months specified in section IV.2.6 should be understood as 60 days counted from the date of submission of tenders. 3. Contractors who are not subject to exclusion from the procedure for awarding a contract pursuant to Art. 24 sec. 1 point 12-23 and art. 24 sec. 5 points 1 Pzp. 4. In order to confirm the lack of grounds for exclusion of the Contractor from the proceedings forordering on the basis of the circumstances referred to in point 3 (Chapter VI, paragraph 1, ToR) The Employer requires the following declarations and documents: 1) A single European order document (EAT) Annex No. 2 to the Terms of Reference (document / y / s) together with the offer, in electronic form), completed in the absence of grounds for exclusion of the Contractor from the provision of grantsordering based on the circumstances referred to in art. 24 sec. 1 item 12-22 iart. 24 sec. 5 point 1 of the PPA Part III, sections A, B, C and D, EAT; 2) excerpt from the relevant register or central records and information about business activities, if separate provisions require entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points 1 pzp(document / s submitted to the ordering party by the Contractor, whose offer was rated the highest); 3) the contractor's statement about the lack of a ruling against him as a preventive measure to prohibit applying for public procurement Annex 4 to the ToR (document / y submitted at the request of the Awarding Entity by the Contractor whose offer was rated the highest); 4) statements by the contractoron the lack of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or - in the event of such a verdict or decision - documents confirming payment of such receivables with any interest or fines or conclusion binding agreement on ppayment of these receivables - Annex No. 5 to the Terms of Reference (document / y submitted at the request of the Awarding Entity by the Contractor whose bid was rated the highest); 5) declaration of the Contractor about belonging or not belonging to the same capital group (Annex 3 to the ToR). In the case of affiliation to the same capital group, the Contractor may submit together with the declaration documents or informationorations confirming that connections with another Contractor do not lead to distortion of competition in the proceedings (document / s submitted within 3 days from the date of placing by the Employer information on the opening of bids on the website (Article 86 paragraph 5 of the PPL).
