Provision of transport services for persons with disabilities

1. The subject of the contract is the provision of specialized transport services for the disabled in the capital city of Warsaw and in the case of treatment or rehabilitation of the disabled in the area up to 20 km from the borders of the capital city of Warsaw.

2. A detailed description of the subject of the contract is included in Annex 1 to the model contract, which is Annex 1 to the ToR.

1. Grounds for exclusion:

Purchaser hatchor from the proceedings of Contractors in relation to which there are premises for exclusion referred to in art. 24 paragraph 1 of the Public Procurement Law (Journal of Laws of 2019, item 1843), hereinafter referred to as the Act.

2. List of statements or documents confirming compliance with the conditions for participation in the procedure and the absence of grounds for exclusion:

a) a single European procurement document;

b) evidence that it is indicated on the formEAT services have been properly performed, i.e.: references or other documents issued by the entity for which the services have been rendered, In this case, justify the inability to obtain documents,

- for periodic or continuous benefits nany references or other documents confirming their proper performance should be issued not earlier than 3 months before the submission deadline;

c) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act;

d) in accordance with par. 9 item 2 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that cane request the Employer from the Contractor in the procedure for the award of a public contract (Journal of Laws of 2016, item 1126 as amended), hereinafter referred to as the "Regulation", the Employer will request the Contractor, which relies on the abilities or situation of other entities on the principles specified in Article 22a of the Act, submit documents referred to in point 13.3.2 (a) of the ToR with respect to these entities.the person representing the entity on whose abilities or situation the Contractor relies, acts on the basis of the power of attorney granted to him, the documents must be accompanied by the power of attorney in one of the forms specified in point 13.1.2 lit. c of the SETC;

e) In the event of a statement by the Contractor in the JEDZ form that a final court judgment has been issued against him or the final administrative decision on arrearstaxes, fees or contributions for social or health insurance The Contractor is obliged to provide documents confirming the payment of these receivables together with any interest or fines or conclusion of a binding agreement on the repayment of these receivables;

f) a statement on belonging or not belonging to the same group company.

3. Other information:

a) Wythe contractor is obliged, before the deadline for submitting bids, to lodge a security deposit in the amount of: 5000 PLN;

b) the term of the bid: 60 days (correction to point IV.2.6 of the announcement);

c) The Employer does not intend to conclude a framework agreement or establish a dynamic system purchase;

d) the Employer will apply the "reverse procedure" described in Article 24aa of the Act to examine and evaluate offers.
