Provision of extended technical support service for the HP Service Manager's environment

The subject of the order is:

1) Provision of extended technical support by the Contractor for the entire HP Service Manager) provided by the Ordering Party.

2) Providing Hewlett Packard software and technical support services under the name: "Hewlett-Packard Technical assistance for BTO software".

3) Providing by the Contractor a document confirming the provision of Hewlett Packard software service and support services under the name: "Hewlett-Packard Pomoc Techniczna do-BTO software ".

4) Performer's modernization of the HP Service Manager environment within the limit of 1 500 man hours.

The detailed description of the subject of the order is specified in Annex 1 to the ToR Description of the Subject of the Order (OPZ) and template Agreement (Annex 2 to the Terms of Reference) . The Contractor will fulfill the condition if he proves that he has financial resources in the amount not less than 170, 000.00 PLN orhas a credit capacity of not less than PLN 170, 000.00.

1. Contractors can apply for the contract, who:

a) are not subject to exclusion, pursuant to art. 24 sec. 1 and par. 5 point 1 of the Public Procurement Law, b) meet the conditions for participation in proceedings regarding: - economic or financial situation

- technical or professional capacity.

2. To be confirmed in the initial confirmationand the conditions for participation in the proceedings and no grounds for exclusion from the proceedings under Art. 25 a para. 1 of the PPL Act, each Contractor must attach to the offer:

2.1. As at the day of submitting bids, a statement, as indicated on the form of the Single European Order Document, prepared in accordance with the specimen of the standard form set out in the executive ordinance of KomiEuropean Union issued on the basis of 59 par. 2 of Directive 2014/24 / EU and art. 80 ust. 3 of Directive 2014/25 / EU, known as "Next" "EAT".

The Ordering Party informs that it has developed an EJP pattern in an electronic form (ESPD).

Instructions on how to prepare an EAT are contained in the SIWZ

2.1.1 In the case of a joint application, EAT shall be submitted each of the Contractors jointly applying for the order (each entity in

3. The ordering party before ordering will call Wykthe tenderer whose tender was rated the most, to submit within the prescribed period, not shorter than 10 days, valid as of the day of submitting the following statements or documents:

3.1 confirming no grounds for exclusion from the proceedings:

- information from the National Criminal Register in the scope specified in art . 24 sec. 1 points 13, 14 and 21 of the Public Procurement Law issued not earlier than 6 months beforethe deadline for submitting bids, - the contractor's statement that he has not issued a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or - in the event of such a verdict or decision - documents confirming the payment of these receivables with possible interest or gor - concluding a binding agreement regarding the repayment of such receivables, - a declaration by the contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders, - a copy from the relevant register or from the Central Register and Information on Business, if separate regulations require entry in the register or record of 3.2 confirming fulfillmentthe terms of participation in the proceedings mentioned in points III.1.1.2 and III.1.3 2. announcements - 4. Contractor within 3 days from the date of posting on the website information referred to in Article 86 para. 5 of the Public Procurement Law, will provide the orderer, without additional request, with a declaration of belonging or non-affiliation to the same capital group referred to in art. 24 sec. 1 point 23 ustawy Pzp. With the submission of a statement, the Contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure. Before the deadline for submitting bids, the Contractor is obliged to pay a deposit of PLN 16, 000.

6. Before signing the contract, the contractor is required to provide security for the proper performance of the contract andprovide copies of certificates of persons referred to in point III.1.3
