1. The subject of the order is the provision of service and technical support along with access to new versions of the Eskulap software and Simple.ERP of the Health Care Team in Brodnica.

The subject of the order is the provision of service and technical support along with access to new versions of the Eskulap software and Simple.ERP of the Health Care Team in Brodnica. A detailed description of how to perform the service is provided in the contract template and the attachment. A detailed description of the subject of the contract, Annex No. 7 of the Terms of Reference.

Zamathe creditor deems the condition to be met if:

the Contractor shows that he has completed within the last 3 years before the expiry of the time limit for the submission of tenders, and if the period of business activity is shorter in that period, at least one service in the field of providing hospital IT service care, o total value not less than PLN 1, 000, 000.00 gross

1. To demonstrate complianceand this condition, the Contractor is obliged to attach to the offer the following documents:

1) a list of performed, and in the case of periodic or continuous services also performed within the last 3 years before the deadline for submission of tenders, and if the period of business is shorter - in this period, together with with their value, subject, dates of execution and entities for whose servicesand have been carried out, and the attachment of evidence indicating whether these supplies or services have been performed or are being performed properly.

2. According to - 2 para. 4 point 2) of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Employer may request from the Contractor in the procurement procedure (i.e. Journal of Laws item 1126 as amended) the evidence is:

1) references or other documentnts issued by the entity for which the supplies or services were performed, and in the case of periodic or continuous services are performed, and if for a justified reason of an objective nature the Contractor is unable to obtain these documents the Contractor`s statement; in the case of periodic or continuous benefits, references or other documents confirming their nSuch performances should be issued not earlier than 3 months before the deadline for submitting bids or requests to participate in the procedure.

Documents submitted by all Contractors in the formelectronic with a qualified electronic signature on the Purchasing Platform until the deadline for submitting offers:

1. Completed and signed Bid Form (print offer) - according to Annex 1 to the ToR;

2. completed and signed Single European Order Document (EAT) according to the model constituting Annex 2 to the ToR. This statement is to constitute a preliminary confirmation that the Contractor nis not subject to exclusion and meets the conditions for participation in the procedure. The Contractor is not obliged to submit declarations or documents confirming compliance with the conditions for participation in the proceedings and no grounds for exclusion, if the Employer has declarations or documents regarding this Contractor or can obtain them using free and publicly available databases, in particular a registerpublic entities within the meaning of the Act of 17.2.2005 on the computerization of the activities of entities performing public tasks (i.e. Journal of Laws of 2019, item 700, as amended). In such a situation, the Contractor is obliged to provide the Employer with a reference number of the procedure in which the required documents or statements are found. EAT must be filled out and provided with a qualified electronic signaturebe submitted electronically before the offer submission deadline;

4. in the case of joint application for the contract by the Contractors, the declaration referred to in point VI.4.2. shall be made by each of the Contractors jointly applying for the contract.

6. list of persons according to Annex 6 to the ToR with attachments;

7. power of attorney, about whichreferred to in point V.2.2.1. ToR (if applicable) ;.

8. a written undertaking to make available the resources referred to in point V.3.1;

9. the justification for reserving the trade secret (if the Contractor reserves the confidentiality of part of the offer - see section X.6.5. of the ToR);

10. proof of payment of the bid security;

11. in case of acting through a proxy, the Contractor is obliged to attach to the offer a power of attorney in the formscan of the original or scan photocopy certified by a notary public and signed with a qualified electronic signature.

Within 3 days from the date of placing on the Employer`s website - https://platformazakupowa.pl/pn/szpitalbrodnica in the tab of the procedure, information from the opening of the offers referred to in art. 86 section 5 of the Public Procurement Law, each Contractor provides the Employer in electronic formwith a qualified electronic signature on the Purchasing Platform, a statement on belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23) of the PPL Act. Along with submitting the declaration, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. declarationthe enclosure should be submitted on the form of attachment No. 3 to the ToR.

Documents confirming that they are not subject to exclusion from the procedure, submitted by the Contractor whose bid was highest rated (in accordance with Article 26 (2) in conjunction with Article 25a (1) of the Public Procurement Law) ). The documents are submitted to the Employer at the request of the Employer addressed to the Contractor with the highest rated offer with the deadline for submittingument shorter than 10 days in electronic form with a qualified electronic signature on the Purchasing Platform.
