The subject of the order is the service of control and monitoring of the implementation of the projectac related to the creation and modernization of digital databases: EGiB, BDOT500 and GESUT for the purposes of the project "Digital geodesy of the geodesic resources of the Lublin province" Stage 2. As a result of this public contract, a supervision and control of surveying works carried out in the Jozefow commune will be selected on the Vistula River (a registration unit of Jozef-w on the Vistula---rural area and a register unit Jozefow nad Wisłą a city) and a cadastral unit Opole Lubelskie Miasto. The subject of the order concerns the project "Digital Geodesy Geodesic Surveying Resources of the Lublin Province", implemented under Priority Axis 2 Cyfrowe Lubelskie, Action 2.1 Cyfrowe Lubelskie, Regional Operational Program of the Lublin Province for the years 2014-2020.

Description of the subject of the order for: record unit Jozefow nad Vistula, rural area (061202_5 Jozefow nad Wisłą rural area), unitand the city of Jozef-w on the Vistula River (061202_4 Jozefow nad Wisłą), the cadastral unit of Opole Lubelskie city (061205_4 Opole Lubelskie city) is set out in Annex 1 to the ToR and documents specifying surveying work carried out under a separate order subject along with the Voivodship Supervision Inspector agreed by the Lubelskie Voivodship Inspector Geodetic and Cartographic projectThe modernization of the land and building register is defined in Annex 2 to the Terms of Reference - Part 67 Opolski District, Jozef-w on the Vistula River Rural Area, Jozef-w on the Vistula River, Opole Lubelskie City. The Contractor shall grant a minimum of 3-year warranty period. Extending the warranty period to max. 5 years is one of the quality criteria described in point 14 of the Terms of Reference. The subject of the orders includes m.in .: verification of a minimum of 100 individual operators constituting PZGiK materials for the controlled commune of Jozef-w on the Vistula (including the register unit of Jozef-w on the Vistula, city, a register unit of Jozef-w on the Vistula, rural area). The number of technical operators that the contractor will accept for verification is also the quality criterion described in point 14 of the SIWZ.

1. For granting-orders may be applied by contractors who are not subject to exclusion in the circumstances referred to in art. 24 sec. 1 and 5 PPL (items 5.4 and 5.5 of the ToR) and meet the conditions for participation in the proceedings specified by the awarding entity in the announcement of procurement and SIWZ

2. The ordering party does not specify a special condition in this respect.

3. According to art. 24 sec. 1 PPL from the proceedings for the order inthe performer is excluded in relation to which any of the circumstances referred to in art. 24 sec. 1 item 12-23 and art. 24 sec. 5 Public Procurement Laws

4. preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the proceedings is a declaration in the form of a Single European Order Document (referred to in the procedure as a "single document").and the contractor sends the ordering party a single document in electronic form with a qualified electronic signature (issued by the qualified trust service provider being an entity providing certification services) and encrypted to the address egeodezja@opole.lublin.pl in such a way that the single document reaches the ordering party before the end of the deadline for the submission of offers. In contentYou should indicate the name and the name of the proceeding in accordance with the instructions for the submission of a single document constituting Annex No. 3 to the ToR.

5. Contractor within 3 days from the date of posting information on the Ordering Party's website, referred to in art. 86 par. 5 of the Public Procurement Law (information from the opening of offers), is obliged to provide the Employer with a statement on prdependence or lack of belonging to the same capital group as referred to in art. 24 sec. 1 point 23 of the Act

6. The contracting authority demands from the contractor, which relies on the abilities or situation of other entities under the rules set out in Article 22a of the Public Procurement Law, present in respect of these entities the documents listed in point 6-7 SIWZ.

7. In the case of contractors jointly applying for thighsNone of them can be excluded due to non-fulfillment of the conditions referred to in art. 24 sec. 1 and par. 5 of the Public Procurement Law, while fulfillment of the conditions for participation in contractor proceedings is demonstrated in accordance with Sections 5 to 6 of the ToR.

8. The ordering party, prior to the order being awarded, will call the contractor whose offer has been evaluated at the most, to submit in the designated, not shorter than 10 days, current dates as at the day of submitting statements or documents confirming the circumstances referred to in art. 25 ust. 1 Public Procurement Law.

9. At the request of the ordering party from art. 26 par. 1 of the Public Procurement Law, the Contractor is obliged to submit the following declarations or documents:

9.1. To confirm compliance with the technical or professional capacity condition set out in point 5.3.3. SIWZ zamthe demanding party requires from the contractor:

a) List of services performed within the last 5 years before the deadline for submitting bids, and if the period of business is shorter - during this period, together with their value, subject, dates of completion and entities for which the services have been performed, and attach proofs specifying whether these services have been duly performed, but the evidence, -referred to are references or other documents issued by the entity for which the services were performed, and if for a justified reason of an objective nature the contractor is unable to obtain these documents - the contractor's statement. The model of the list of services is Annex No. 6 to the ToR.

Further required documents are indicated in the next part.

The ordering party does not specify the condition in the above-mentioned-scope.

Continuation of entries in point III.1.3:

b) List of persons, directed by the contractor to perform the public order, responsible for providing services, along with information on their professional qualifications, experience and education necessary to perform the public order, as well as the scope of activities performed and information with a basis to dispose ofpeople. A model list of persons referred by the contractor for the implementation of the public procurement is attached as Appendix 7 to the ToR.

10. In order to confirm the absence of grounds for excluding the contractor from participation in the proceedings, the contracting authority requests the following documents:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 PPL and, in respect of convictions for offenses against-the penalty of arrest, to the extent determined by the ordering party under art. 24 sec. 5 point 5 and 6 of the PPL, issued not earlier than 6 months before the deadline for submitting bids;

2) a certificate from the competent head of the tax office confirming that the contractor is not in arrears with the payment of taxes, issued not earlier than 3 months before the deadline for submission of tenders, or another dokumentu confirming that the contractor has concluded an agreement with the competent tax authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained legally permissible, postponement or installment payment of overdue payments or suspension in full of the decision of the competent authority; the appropriate local organizational unit. ZaSocial Insurance Fund or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of these amountswith any interest or fines, in particular obtained legally required exemption, postponement or installment payment of overdue payments or suspension in full of the decision of the competent authority; - 4) excerpt from the relevant register or central records and business information, if separate provisions require entry in the register or records in order toconfirmation of the lack of grounds for exclusion under Art. 24 sec. 5 point 1 Pzp;

5) the contractor's statement on the lack of issuing a final court judgment or final administrative decision on default of payment of taxes, social security or health insurance fees or - in the event of such a verdict or decision - documents confirming the payment youreceivables with possible interest or fines or conclusion of a binding agreement regarding repayment of such receivables; - 6) declaration of the contractor about the lack of a ruling against him as a preventive measure of prohibition of applying for public orders; - 7) declaration of the contractor about the lack of a final conviction judgment judgment for offenses imposed on the restriction of liberty or a fine inlimit specified by the awarding entity pursuant to art. 24 sec. 5 points 5 and 6 Pzp;

8) the contractor's statement about the lack of a final administrative decision against him under breach of labor law, environmental law or social security regulations to the extent specified by the awarding entity under Art. 24 sec. 5 point 7 Pzp;

9) statements fromContractors about not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716),

If the contractor indicates the availability of declarations or documents referred to in - 5 (submitted to confirm the lack of groundsexcluding the contractor from participation in the proceedings) and -7 (submitted to confirm the absence of grounds for excluding the contractor from participation in the proceedings by the contractor having its registered office or place of residence outside the territory of the Republic of Poland, referred to in point 6.5 of the ToR) of the Regulation of the Minister of Development of 26.7 .2016 on the types of documents that the contracting authority may request from thein the proceedings for the award of a contract (Journal of Laws of 2016, item 116) in electronic form under specific Internet addresses of freely available and free databases, the orderer retrieves declarations or documents indicated by the contractor himself from these databases. If the contractor indicates declarations or documents referred to in - 5 (submitted in order to confirmno grounds for excluding the contractor from participation in the procedure) and - 7 (submitted to confirm the absence of grounds for excluding the contractor from participation in the proceedings by the contractor having its registered office or place of residence outside the territory of the Republic of Poland, referred to in point 6.5 of the ToR) of the Regulation of the Minister of Development of 26.7.2016, which are in the possession of the ordering party, in particular declarations or documents held by the ordering party in accordance with art. 97 ust. 1 Pzp, the ordering party to confirm the circumstances referred to in art. 25 ust. 1 point 1 and 3 PPL, make use of possessed statements or documents, if they are valid.

19. Documents prepared in a foreign language must be submitted together with a translation into Polish. In the case referred to inpoint 7.24. The SIWZ contracting authority may request from the contractor to provide a translation into Polish specified by the contractor and documents collected by the contracting authority

20. The Contractor is obliged to submit a bid bond in the amount of PLN 3, 000.00 (in words: three thousand zlotys).

21. The period of two months specified in section IV.2.6) should be understood as 60 days counted from the date of submission of the offerert.

22. Detailed information on the conditions of participation in the proceedings are included in point 5, 6 and 7 SIWZ.

23. The Employer does not provide for the possibility of ordering within three years from the date of ordering the basic, existing contractor relying on similar services, consistent with the subject of the order.
