The subject matter of the orders are serviceson maintenance and current maintenance of road lighting equipment for selected intersections (road junctions) and sections of national roads located on the national roads of the Małopolska province, divided into three parts.

Main Site: Wojewodztwo małopolskie, Krak-w Area

Part 1 covers maintenance and ongoing maintenance of lighting equipment road of selected intersections (road junctions) andnational road sections located on the national roads of the Małopolska province, administered by the Krakow District. - The scope of the service includes devices located on roads DK A4, DK S7, DK 7 and DK 94.

Szczegłowo, the scope of the order is specified in Volume II-IV of the Terms of Reference. NOT APPLICABLE

1. The Contractor is obliged to submit the current declaration as of the day of submitting the offersconfirmation that the Contractor:

a) is not subject to exclusion;

b) meets the conditions for participation in the proceedings.

2. The certificate referred to in item 1 above (in the form of a single European order document prepared in accordance with the standard form provided for in the Regulation An executive order of the European Commission on the basis of Article 59 (2) of Directive 2014/24 / EU, hereinafter referred to as'a uniform document "or" EAT "), the Contractor is obliged to send the Ordering Party in an electronic form with a qualified electronic signature, in accordance with the rules specified in IDW.

3. In terms of "Part IV: Eligibility Criteria" EAT, the Contractor may confine himself to completing the section, in which case the contractor does not complete any of the other sections (A-D) in Part IV.4. The Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that he refers to their resources, the condition of participation in the proceedings, submits uniform documents also concerning these entities. with the information contained in the EAT, if it is relevant to the specific abilities of the contractor, please specifyeży attach for each of the entities to which the information required in Part IV, GOOD

5, applies. In the event of a joint application by the Contractors, a single document in electronic form bearing a qualified electronic signature shall be submitted by each of the Contractors jointly applying for the order. They should be sent in accordance with the rules set out in IDW

6. On callThe Ordering Party, in order to confirm the lack of grounds for excluding the Contractor from participation in the proceedings, the Contractor is obliged to submit information, certificates, excerpt and statements referred to in - 5 points from 1 to 6 and 9 of the Regulation of the Minister of Development dated 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the proceedings for the award of the contract (Dz.U.-from 2016, item 1, 126.)

7. If the Contractor has his registered office or place of residence outside the territory of the Republic of Poland, and also if the person whose document relates is residing outside the territory of the Republic of Poland, the principles set out in - 7 and 8 above apply. Regulation.

8. Contractor, within 3 days from the date of posting on the websitereferred to in art. 86 par. 5 of the Public Procurement Law, shall provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. From the proceedingsthe ordering entity is excluded in relation to which any of the circumstances referred to in art. 24 sec. 1 point 12 of the Public Procurement Law and art. 24 sec. 5 points 1-4 and 8 of the Public Procurement Law

10. The term of the Service will be 36 months from the date of signing the Agreement.

11. The ordering party, in accordance with art. 24 aa of the Public Procurement Law, provides for the possibility of first of all assessing the offert, and then examine whether the Contractor whose tender has been rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure.

12. The period of being bound by the offer referred to in point IV.2.6) of the announcement is 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires. The ordering party provides for the possibility of ordering, about whichwa in art. 67 sec. 1 point 6 uPzp within the scope described in the SIWZ.
