The subject of the order is a comprehensive, round-the-clock service performing services consisting in transporting medical patients of the University Hospital in Kradoctors requiring the presence of a doctor together with medical staff of the S-type ambulance and the physicians who are not required by the paramedic and medical rescuer. P

The subject of the order is a comprehensive, 24-hour service consisting in transporting medical patients Hospital Uniwersin Cracow, requiring the presence of a physician together with medical staff, S-type ambulance, and the presence of a paramedic and paramedical ambulance driver.

The detailed description of the order is included in Appendix No. 5 to the specifications (including attachments to this description). This description should be read along with any changesand the contents of the specifications, being eg the result of responses to contractor inquiries.

The estimated number of services within 36 months, specified in Annex 1a to the specification, is only the Employer's estimate and is used to calculate the offer price (ie determine the contractor's maximum remuneration). The contractor will be obliged to provide the service in accordance with this specification.

ZThe conditioner will not meet the condition in this respect. - The condition will be met if the contractor is insured against civil liability in the scope of business related to the subject of the order, for the sum insured (guarantee sum) not less than PLN 600, 000.00. participation in the proceedings also pursuant to art. 24 sec. 5 points 1 and 8 of the Act - List-statements or documents confirming compliance with the conditions of participation in the proceedings and no grounds for exclusion:

1. Declaration in the scope indicated in Attachment No. 2 to the specification. The information contained in the statement will constitute preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the proceedings.

The statement must be in the form of an electronic document, with a qualified electronic signature, prepared and transmitted to the Ordering Party using means of electronic communication within the meaning of the Act of 18.7.2002 on the provision of electronic services. A detailed description of the preparation and submission of the document can be found in point 10.4 Specifications. 2. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, -14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids;

3. certificate of the competent US governor confirming that the contractor is not in arrears with paying taxes, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the contractor has concluded an agreement with the competent tax authority in the casethe lats of these receivables, together with possible interest or fines, in particular obtaining the legally required exemption, postponement or spreading into installments of overdue payments or suspension of the execution of the decision of the competent authority in full;

4. certificate of the relevant ZUS or KRUS field organizational unit or other document confirming that the contractor is not in arrears with paying contributionsfor social or health insurance, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the contractor has concluded an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtained the legally required exemption, postponement or spreading overdue payments into installmentsand or the suspension in full of the implementation of the decision of the competent authority;

5. excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Article. 24 sec. 5 point 1 of the Act;

6. the contractor's statement that he did not pass a final court judgment against himor a final administrative decision on default of taxes, fees or social security or health insurance premiums or, in the event of such a judgment or decision being issued, documents confirming the payment of such claims together with any interest or fines or a binding agreement on repayment of these debts.

7. statements by the contractor aboutto a ruling against him as a preventive measure for the prohibition of applying for public orders.

8. statements of the contractor on the non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716).

9. list of services performed within the last 3 years before the deadline for submission of tenders, and if the periodbusiness activities are shorter - in this period, along with their value, subject, dates of completion and entities for which services were performed, prepared according to the template constituting Annex 4 and evidence determining whether these services were performed or are performed properly.

10. documents confirming that the services offered meet the requirements in accordance with point 6.5.10.1. spec- The list for contractors established outside the RP specifies the specification. - The form of the power of attorney (original or notarized certified copy) is set out in point. 10.2.4. specification.

The ordering party does not provide for the order for which referred to in art. 67 section 1 point 7 of the Act
