Part I - Photovoltaic installations: Designing (for each facility a separate project), delivery, assembly, commissioning and implementation of the procedure for including solar photovoltaic installations in the DSO network along with instructions for users facilities for servicing installations and servicing installations during the periodarance and warranty. Photovoltaic installations generating electricity will be installed and used for social and living needs in 118 individual households.

Main Site: Pierzchnica Commune and Wodzisław Commune, POLAND

Designing (for each facility a separate project), delivery, assembly, commissioning and implementation of the procedure for including the photographic micro-installation into the OSD networkltaicznych along with instructing users of facilities in the field of installation service and servicing the installation during the warranty period and warranty. Photovoltaic installations producing electricity will be installed and used for social needs in 118 individual households. A. Wodzisław Commune:

1. Design, delivery and assembly:

1) photovoltaic installations producing electricity with an optimum estimated power of 1 PV panel, minimum 270 Wp in 66 individual households.

2. Installations with a total capacity of minimum 221.4 kWp (0.22 MWe) will be located on the property of the residentin Wodzisław Commune a total of 66 objects:

a. with a capacity of 2.16 kWp, 8 modules, 6 units of installation,

b. with a capacity of 3.24 kWp, 12 modules, 34 units of installation,

c. with a capacity of 3.78 kWp, 14 modules, 26 units of installation

3. Depending on the technical conditions, it is planned to install the installation on the roofs of residential buildings (49 installations), the roof of the farm building adjacent to the building(1 installation), rooftops of free-standing farm buildings (7 installations), elevation of free-standing farm building (1 installation) or on the ground (8 installations).

4. Organizational and training works - a) preparation of the installation operation manual,

b) training of property owners from the principles of operation, use, maintenance and safety related to the use of the installationphotovoltaic installation,

c) drawing up a report on the training, detailing what was the subject of the training and providing instructions,

5. Prior to the implementation of the task, the Contractor will be required to carry out all the necessary expertises, inspections, local visions and verify the information on the delivery and assembly of the complete photovoltaic installation(1) if required by the provision, an application for issuing connection conditions for the power grid with the necessary documents should be prepared, 2) micro-installations should be reported to the Power Grid.

B. Pierzchnica commune:

1. The subject of the orders includes the design, delivery and assembly of:

1) photovoltaic installations producing electricity for optimal use1 PV panel, minimum 270 Wp in 52 individual households.2. Installations with a total capacity of minimum 170.1 kWp (0.17 MWe) will be located on the property of the residents of the Commune a total of 52 objects:

a) with a capacity of 2.16 kWp, 8 modules, 8 units of installation;

b) with a capacity of 3.24 kWp, 12 modules, 25 units of installation; c) with a capacity of 3.78 kWp, 14 modules, 19 units of installation

3.Depending on technical conditions, it is planned to install the installation on roofs (36 installations) or fa-ades (2 installations) of residential buildings, free standing farm buildings (11 installations) or on the ground (3 installations).

4. Organization and training work

1) preparation of installation operation manual,

2) training of property owners from the principles of service, useda) maintenance and safety related to the use of the installed photovoltaic installation,

3) preparation of a report on the training, detailing what was the subject of the training and providing instructions.

5. Prior to the implementation of the task, the Contractor will be required to perform all the necessary expertise, visual inspection, local visions and verify the information regardingcarrying out the delivery and assembly of a complete photovoltaic installation. 1) if required by the provision, an application for issuing connection conditions for the power grid with the necessary documents should be prepared

2) micro-installations should be reported to S. Elektr. • The ordering party does not specify requirements in this respect.

1.Our contractors may apply for the contract, who:

1) nie are subject to exclusion;

a) art. 24 sec. 1 point 12 23 of the Act

b) of art. 24 sec. 5 point 1) The contractor must attach to the offer an up-to-date statement as per the day of submitting bids in the scope indicated in the SIWZ. The declaration shall be made on the form of the Single European Order Document (EED), drawn up in accordance with the specimen of the standard form set out in the Commission Implementing Regulation.issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU and art. 80 par. 3 of Directive 2014/25 / EU. The information contained in the EED will be a preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the procedure. Both EAT and the offer are made exclusively in electronic form. The ordering party, before granting the order, will call the Contractor whose offer will beconstant highest rated, to be submitted within the prescribed period, not shorter than 10 days, valid as at the date of submitting the following statements or documents:

6.1. To confirm the absence of grounds for excluding the contractor from participation in the proceedings:

1) excerpt from the relevant register or from the Central Register and Information about Business, if separate regulations require an entry in the register or ein order to confirm the absence of grounds for exclusion under Article 24 sec. 5 points 1 of the Act; applies to: excerpt from the National Court Register (for entities entered into the National Court Register) or a copy from the Central Register of Economic Activities (for entities entered into CEDIG) indicated registers are generally available and free databases, so the contracting authority will collect information from thesebases. In order to confirm the lack of grounds for excluding the contractor from participation in the proceedings: information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids.

2) a certificate from the competent head of the tax office confirming that the contractor is not in arrears with paying taxes, not earlier than 3 months before the deadline for submission of bids, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular has obtained the legally required exemption, postponement or distribution of overdue installments payment or suspension in fulla decision of the competent authority - 3) certification of the relevant organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of offers, or other pot documentthat the contractor concluded an agreement with the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained legally permissible, postponement or installment payment of overdue payments or suspension in its entirety of the decision of the competent authority. failure to issue a final court judgment against him or herthe final administrative decision on default of taxes, fees or social security or health insurance premiums, or - in the event of such a verdict or decision - documents confirming the payment of these claims together with any interest or fines or conclusion of a binding agreement regarding repayment of such claims, 5) the contractor's statement about the lack of oto him as a preventive measure for the prohibition of applying for public orders.

6) the contractor's statement on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716) - the Contractor will meet the condition, if demonstrates that: he is insured against civil liability in the scope of the conductedactivities related to the subject of the order, the insurance amount should not be less than: for part I PLN 1, 000, 000 (say: one million 00/100); for part II PLN 1, 000, 000.00 (in words: one million 00/100); In order to confirm the fulfillment of the condition, the contractor will be obliged to submit on the ordering party's request a document confirming that the contractor is insuredfrom civil liability in the scope of conducted activities related to the subject of the order for the guarantee sum specified by the ordering party in Chapter V.1 2) b) ToR.

The Contractor will submit with the offer:

a) Power of attorney to represent the Contractor in this proceeding and / or to sign contract (unless it is clear from the registration documents). The power of attorney must be signed by the personmay be authorized to represent the Contractor (qualified signature / signatures of persons granting power of attorney) or take the form of a notarized deed, or a notarized copy, or a copy certified as a conformity to the original in a manner consistent with the Regulation of the Minister of Development of 27.7.2016 on types of documents which the contracting authority may demand from the contractor in the proceedings for grantingNo order (Journal of Laws of 2016, item 1206 as amended)

b) Proof of payment of deposit if it is made in a form other than cash. If the contractor has a registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to:

a) in point 6.1 point 1) SIWZ - provides information from the relevant register or, in the absence of such a register, another document confirmsthat if the liquidation has not been opened or bankruptcy has been opened,

- If the country in which the contractor has its registered office or place of residence has the person concerned, the documents referred to in point a), it is replaced by a document containing the contractor's statement, indicating the person or persons authorized to represent him or her, orb statement of the person to whom the document was to apply, submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the seat or place of residence of the contractor or place of residence of that person.

c) in point 6.2 point 2) and 3) SIWZ - submit a document or documents issued in the country in which the contractor has its registered office or place of residence, confirming that it is not in arrears with taxes, fees, social security or health insurance contributions or that it has entered into an agreement with the competentthe authority regarding the repayment of these receivables, together with any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension in full of the decision of the competent authority - issued not earlier than 3 months before the deadline for submission of tenders.

d) Contractor having its registered office in the territory of the Republic of PolandPoland, in relation to a person residing outside the territory of the Republic of Poland, which is referred to in the document indicated in point 6.2 item 1), submits the document, to the extent specified in art. 24 sec. 1 points 14 and 21 of the Act. If in the country where the person whose document is to be residing in the place of residence does not appear such documents, it is replaced by a document containing the educationthe person is attached to a notary or before a judicial, administrative or professional body competent for the place of residence of that person. The document must be issued not earlier than 6 months before the deadline for submission of offers.
