The subject of the order is a comprehensive delivery-and implementation of the Hospital Integrated IT System and the Internet Patient Portal (e-service) with a training module and delivery of computer devices with software within the framework of the project: "Informatisation and development of electronic services in the Neuropsychiatric Hospital. Professor Mieczysław Kaczyński SPZOZ in Lublin ", co-financed under the Regional Operac Programof the voivodship of Lublin in the years 2014-2020 ".

The subject of the order is comprehensive delivery and implementation of the Hospital Integrated Information System and the Internet Patient Portal (e-service) with the training module and delivery of computer devices with software within the framework of the project: "Informatization and development of electronic services"who are in the Neuropsychiatric Hospital Professor Mieczysław Kaczyński SPZOZ in Lublin ", co-financed under the Regional Operational Program of the Lublin Province for the years 2014-2020". A detailed description of the subject of the order is attached as Annex 1 (1a and 1b) to the ToR.

I. In order to confirm that the contractor is not subject to exclusion pursuant to art. 24 sec. 1, art. 24 sec. 5 point 1 of the Act and meets the conditions for participation in the proceedings, referred to in art. 22 ust. 1b of the Act should be submitted:

- valid as of the day of submitting bids - a declaration made pursuant to art. 25 a para. 1 and 2 acts in the form of a uniformo the European order document EAT according to the template prepared in accordance with the standard form specified in the Commission Implementing Regulation (EU) 2016/7 of 5.1.2016 laying down the standard form for a single European order document (OJ LUL 166.3.16) constituting Annex 4 to SIWZ, according to the following rules:

EAT should be sent in the form of ele-mailed with a qualified electronic signature. The statements of entities submitting a joint offer and entities providing the potential submitted on the EAT form should take the form of an electronic document signed with a qualified electronic signature by each of them to the extent that they confirm the circumstances referred to in Article 22 para. 1 of the Act. Particularg. Ch. VII SIWZ.

II. The ordering party will call the contractor whose offer has been rated the most, to submit in the prescribed, not shorter than 10 days, the current date as of the day of submitting the following statements or documents confirming the lack of grounds for excluding:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act issued not earlierless than 6 months before the deadline for submission of tenders (in the form of the original or a copy certified as a true copy by the Contractor);

2) statements of the contractor about the lack of a final court judgment or final administrative decision on default of taxes, fees or contributions for social or health insurance or in the event of such a judgmentor decisions of documents confirming payment of these receivables together with any interest or fines, or conclusion of a binding agreement regarding repayment of such debts (in the original form);

3) declaration of the contractor about the lack of a ruling against him as a preventive measure of prohibition of applying for public orders (in in the form of the original);

4) excerpt from the relevant register orb from central records and information about business activities, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act (in the form of the original or a copy certified as a true copy by the Contractor). III. The ordering party will call the contractor whose offer has been rated the most, to submit inmarked, not shorter than 10 days, the current date as at the day of submitting the following statements or documents confirming fulfillment of the conditions of participation in the proceedings: - 2.1 List of services performed, according to the template constituting Annexe No. 6 to the ToR (in the original form),

2.2 List of persons, by the form constituting Annex 7 to the ToR (in the form of the original).

IV. The bid security has been defined in Chapter. X SIWZ.

V. The Contracting Authority requires a performance bond to be provided in the amount of 10% of the gross contractual remuneration presented in the offer (Chapter XVII of the ToR).

VI. Based on Article. 93 para. 1a of the Act, the Orderer may annul the procedure for awarding the contract, if the funds from the budget of the European Union, which the Employer intends to spend on the implementation of the contractthey will not be awarded.
