The subject of the orders is the construction and implementation of the e-services platform at the Independent Public Health Care Center in Parczew within the framework of the project "Development of e-services in SP ZOZ in Parczew "divided into 3 tasks described in detail in Annexes 3a, 3b and 3c to this ToR. The order is co-financed under the project" Development of e-services at SP ZOZ in Parczew "No. RPLU.02.01.00-06-0017 / 17 as part of the Regional Operational Program of the Lubelskie Voivodeship for the years 2014-2020, Priority Axis 2 Cyfrowe Lubelskie, Activities 2.1 Cyfrowe Lubelskie. Ordering tohe is willing to submit partial offers. When submitting an offer, each Contractor is obliged to pay for the offered tasks a bid bond in the amount of: Task No. 1 - PLN 12, 000.00, in words: PLN 12, 000; Task No. 2 - PLN 12, 000.00, in words: PLN 12, 000; Task No. 3 - PLN 7, 000.00, in words: PLN 7, 000. Main Site: Independent Public Health Care Center in Parczew, ul. church-136, 21-200 Parczew, POLAND. Task 1 includes the delivery of the server environment with software and network modernization. The Employer will exclude from the Contractor's proceedings: 1) who have not demonstrated fulfillment of the conditionsdepartment in the proceedings; 2) who will not prove that they do not have the conditions specified in art. 24 sec. 1 item 13-23 of the PPL; 3) for which the conditions specified in art. 24 sec. 5 points 1, 2, 4 and 8 PPL.
