The subject of the order is:

Main Site : The services will be provided in two ZUS calculation centers located in Warsaw.

The object of the contract is:

1. providing maintenance servicesinput (SWMA) of the following IBM Db2 Connect software in the range below:

1.1. IBM Db2 Connect Unlimited Edition for System z for Linux on with Host Server 5,

1.2. IBM Db2 Connect Unlimited Edition for System z for Linux on with Millions of Service Units 2152 MSU.

2. rendering by the Contractor to the Employer consulting services not exceeding 80 man-hours.

III. Other additional information:

1. In the event of an appeal, the appellant sends a copyappeal by ordering by e-mail only to the following address: SekretariatDZP@zus.pl The offer binding period set out in section IV.2.6 of this notice is 90 days.

3. The Contractor submitting the offer is obliged to pay a deposit in the amount of PLN 50, 000.00 (in words: fifty thousand zlotys).

4. Amendments to the provisions of the contract have been specified in the model contract (Annex 2 to the ToR).
