1. The subject of the order is the Installation of the line monitoring system.

Main Site: The seat of the Ordering Party Konstancin-Jeziorna and other locations in the Republic of Poland

1 The subject of the order is Installation of the line monitoring system.

2. Opis subject of the order is included in Part II of the ToR (Description of the subject of the contract) and in Part IV of the ToR (Essential provisions of the Contract)

3. Pursuant to the provisions of art. 29 para. 3a of the Act on Public Procurement Law, the Employer requires that persons directed to provide the service, i.e. persons performing: - assembly and disassembly of measuring points on columns, - preparation of a detailed assembly design project-measuring points on poles,

were employed on the basis of an employment contract for the duration of the concluded contract. The requirements set out in Article 36 par. 2 point 8a of the Public Procurement Law Act are specified in the model contract constituting Part IV of the ToR. - The Contractor has financial resources or creditworthiness in the amount not less than PLN 1, 000, 000.00 (in words: one million zlotys). In the case of demonstration-with financial assets or creditworthiness, in a currency other than PLN, the conversion of these values will take place at the average exchange rate published by the National Bank of Poland on the day of publication of the contract notice in the Official Journal of the European Union. In the event that on this day the National Bank of Poland (table A) does not publish the average rate table, the Ordering Party will accept as a basis the rate from the table of average rateon the first day after the date of publication of the contract notice in the Official Journal of the European Union.

1) In order to confirm the fulfillment of the conditions for participation in the procedure and the absence of grounds for exclusion specified by the Employer in the Notice of Procurement, the Economic Operator submits an application for participation in the proceeding (hereinafter: application) valid as of the date of lodgingiosku statement in the form of a single document (EED), in accordance with the standard form specified in the EC Implementing Regulation (EU) 2016/7 of 5.1.2016 laying down a standard form for a single European order document.

2) The contractor is obliged to attach on the Platform Purchase electronic uniform document / y (EAT), referred to in point VII in accordance withe) Model No. 2 Part III of the SIWZ.

3) The single document (s) should be drawn up in Polish and kept in electronic form and signed with a qualified electronic signature.

4) In the case of joint application by the Contractors, a single document shall be submitted by each of the Contractors jointly applying for the order. The single document confirms fulfillment of the conditions for participation in the proceedingsiu, selection criteria and no grounds for exclusion to the extent that each of the Contractors demonstrates fulfillment of the conditions for participation in the procedure, selection criteria and no grounds for exclusion.

5) In the case where the Contractor relies on the financial situation, technical or professional abilities, other entities The Contractor submits with the application evidence that the Contractor will have the necessary ones-resources to the extent enabling the proper implementation of the public order and the assessment of whether the relationship between the contractor and these entities guarantees real access to their resources. The evidence shall specify in particular: (1) the scope of the Contractor's resources of another entity, (2) the manner of using the resources of another entity by the Contractor, when executing the order, (3) the scope and period of participation of another entity-of the entity when executing the order, (4) whether the entity, on the capacities of which the contractor relies with respect to the conditions for participation in education, professional qualifications or experience, will provide services whose indicated abilities apply. This proof is in particular the written obligation of other entities to provide the Contractor with the necessary resourcesand the period of using them when executing the order. The Contracting Authority informs that these documents may be subject to clarification in order to obtain the required information.

6) In order to confirm the lack of grounds for exclusion indicated in art. 24 sec. 1 point 23 of the Act, the Contractor, within 3 days from the day of submitting the information referred to in art. 57 par. 1 of the Act, it passes to the Ordering Party (without separate se) declaration of belonging or non-affiliation to the same capital group. With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. In the case of Contractors jointly appearing the statement referred to in item 1, each Contractor submits.
