The subject of the order is: service consisting in the liquidation of boreholes August 23, 55, 61, Brelikow 19, 45, 68, 71, 116, Emilia 14, Gorlice 2, 8, Gubernia 2, Księżpol 10, Leszczowate 46, Nosowka 10K, Paszowa 15, Przemyśl 213, Ropienka 97, Stężyca 9 on hydrocarbon deposits for PGNiG SA Sanok Branch with all necessary work to complete the task. The subject of the order is: service consisting in the liquidation of wells August 23, 55, 61, Brelikow 19, 45, 68, 71, 116, Emilia14, Gorlice 2, 8, Gubernia 2, Księżpol 10, Leszczowate 46, Nosowka 10K, Paszowa 15, Przemyśl 213, Ropienka 97, Stężyca 9 on hydrocarbon fields for PGNiG SA Sanok Branch with all necessary work necessary to complete the task. the works include, among others:

preparatory work,

- assembly of the device,

- kneading the charging,

- execution of cement plugs,

- cement reta clasp, iner,

- RBT measurements,

- well filling with a bentonite scrubber,

- cutting out and pulling out 5 ", 5 1/2", 7 ", 8", 9 ", 10" and 12 "pipes,

- liquidation of the borehole,

- disassembly of the device,

- waste utilization,

- cleaning works.

The contracting authority requires that work be carried out simultaneously, with five devices on five wells. Due to the nature of the order, the Ordering Party wishes to liquidate allfor indicated wells by one selected Contractor. For this reason, he did not divide the order and does not allow the submission of a partial offer. The contractor selected for execution must have a liability insurance policy for the duration of the contract for the amount of PLN 2 000 000 for the duration of the contract., the manner and conditions of its execution and the requirements of the Employer are presented in Annexes 1.1.1.1 to the ToR.

The Employer does not specify the condition for participation in the procedure.

The Contracting Authority does not specify the condition for participation in the proceedings. Under the offer binding period, two months should be understood as 60 days.

2. Contractor, within 3 days from the date of posting on the website information from the opening ofert submits (via the Electronic Communications Module with Suppliers of PGNiG SA) a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23. Together with the submission of the statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the Proceedings - prepared according to the formula constituting theswitch 4 to SIWZ.

3. The Bid, together with attachments, is submitted by the Contractor via the Electronic Communication Module with Suppliers of PGNiG SA

4. The contents of the offer are the documents listed in point 12.6 SIWZ.

5. In the absence of grounds for exclusion, the Ordering Party will call Contractors, whose offers will be evaluated as the most advantageous, to submit within the prescribed period, not shorter than 10 days, as of the date of submission, appropriate declarations or documents confirming the lack of grounds for exclusion from the proceedings referred to in par. 5 points 1-9 of the Regulation on the types of documents that the Employer may request from the Contractor in the proceedings for the award of the contract and indicated in the ToR.

6. The contracting authority provides for the application of the procedure described in art. 24aa Pzp.
