Providing the services necessary to carry out the drilling of the probe works for the Przemyśl 299K and Przemyśl 308K holes. Reference Number: ZP / PGNG / 18/0068 / GE / DWR

The subject of the order is: Provision of services necessary for completion of the drilling stage for probes for Przemyśl 299K and Przemyśl 308K wells. The scope and description of the object are detaileda, the manner and conditions of its execution and the requirements of the Ordering Party are presented in Appendix No. 1 to the ToR, respectively for each part of the order (from 1.1 to 1.17.)

The Ordering Party considers the condition to be fulfilled if the Contractor demonstrates that he will have valid decisions and permits to conduct business issued in accordance with applicable regulations, in particular resulting from the Act of 27.4 .2001 Environmental Protection Law (Journal of Laws of 2018 item 799), Act of 2September 7, 2001 on waste (Journal of Laws of 2018 item 992), the Act of November 10, 2008 on mining waste (Journal of Laws of 2017, item 1849), Act of 20.07.2017 - Law waters (Journal of Laws 2017 item 1566)

The ordering party does not specify the condition of participation in the proceedings • The offer should be accompanied by the current uniform European order document as described in point 8.2 of the ToR. The ordering party will call outthe bidder whose offer was rated the highest, to submit within the prescribed period, not shorter than 10 days, valid as at the date of submission, statements or documents confirming compliance with the conditions of participation in the proceedings referred to in the ToR. In the scope of the lack of grounds for exclusion, the Purchaser, before choosing the best offer, will call the Contractor whose offer was the higheston, to submit within the prescribed period, not shorter than 10 days, valid as at the date of submission, appropriate declarations or documents confirming no grounds for exclusion from the proceedings, referred to in par. 5 points 1-9 of the Regulation on the types of documents that may be requested by the Awarding Entity from the Contractor in the proceedings for granting the order and indicated in the Terms of Reference. Contractor, on time-3 days from the date of placing by the Employer of the information on the website referred to in art. 86 par. 5 of the Public Procurement Law, will provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 11 of the Public Procurement Law. In the case of belonging to the same capital group, the Contractor, together with submitting a statement, may present the evidencethat connections with another Contractor do not lead to distortion of competition in the Procurement Procedure. • Under the bidding period, 2 days should be understood 60 days.

Offer contents: Offer prepared in accordance with the offer form constituting Appendix No. 2 to the Terms of Reference, price forms prepared in accordance with Annex No. 3 to the ToR - Statement in the form of EAT (submitted in the manner specified in clause 8.4 of the ToR)), - Obligation of another entity to place the Contractor at the disposal of the necessary resources for the implementation of the Order (in the situation where the Contractor relies on the ability or the situation of other entities), in the form of the original; - Where Contractors jointly apply for granting the Order, they are obliged present a document establishing the proxy authorized to represent in the posteither to represent or represent in the Proceeding and to conclude an Agreement on the Order;

Technical description of the order for the parts: 2, 4, 13, 14, 15, 17 in which it will present the manner of execution of work performed under this order, based on information contained in Annex 1 to the ToR for individual parts. In the description of the method of implementation of the works, unless in technical requirements, no-stated otherwise, the Contractor will describe how and with what tools and equipment he will carry out the work. The Contractor will also present a short description of the materials used. The Contractor can independently describe the parameters or present other materials, e.g. diagrams that will confirm that the subject matter of the order meets the requirements set by the Ordering Party.The applicant provides for the application of the procedure described in art. 24aa Pzp.

The contractor who submits an offer for cementing service (part 5) at the location covering the subject of the order can not provide cement leaven service (part 6) on this site. - Contractor who submits an offer on the site of loading, transport and processing of waste removal ( part 3) must submit before signing-contract, certificate of non-transfer of the entity with payment of fines for violation of environmental protection requirements issued by the competent Voivodship Inspectorate of Environmental Protection, issued not earlier than three months before the deadline for submission of tenders.
