Purchase of DSNG satellite station for the Local Department of TVP S.A. in Białystok

Reference Number: ZP / TXCC / 1/2018

1. The subject of the order is the purchase of DSNG satellite station for the Regional Department of Polish Television in Białystok consisting in the delivery of a new caron the basis of a FURGON type vehicle with a maximum gross weight of 5, 0T and execution of its specialized development for the needs of DSNG Station operating via distribution satellites of operators such as Eutelsat, Intelsat, SES Astra, Sirius, Amos, Hellasat and others, and disassembly of existing equipment A / V from the currently used DSNG car and mounting them on a new carand not according to Annex 10 to the ToR. 2. A detailed description of the subject of the order is provided in Annex No. 1 and 10 to the ToR.

1. The subject of the order is the purchase of DSNG satellite station for the Regional Department of Polish Television in Białystok consisting in the delivery of a new technological vehicle based on a FURGON type vehicle with a permissible total weight of 5, 0T and execution of its constructionSpecialist for the needs of DSNG Station working via distribution satellites such operators as Eutelsat, Intelsat, SES Astra, Sirius, Amos, Hellasat and others and performing disassembly of existing A / V devices from the currently used DSNG car and installing them on the new car in accordance with Annex No. 10 to SIWZ. 2. A detailed description of the subject of the order is provided in Annex No. 1 and 10 to the ToR.

1 Declarations and documents submitted with the offer:

1) Offer form, in accordance with Annex 2

2) Price specification in accordance with Att. 2A

3) documents confirming the entitlement to sign the offer and to sign or certify for compliance with the original statements or documents If separate regulations require an entry in the register or records, the appropriate document is an extract from the relevant register or records,

4) power of attorney, if the offer or attached to it statements or documents is signed by the attorney

5) document confirming the payment of the bid security deposit in the non-cash form

6) EED submitted in electronic form in accordance with the rules described in 9.3.6SIWZ,

7) evidence confirming that the Contractor will have at his disposal the necessary resources of others under the orderlitter, which is in particular the obligation of these entities to provide the Contractor with the necessary resources for the order

2 Declarations or documents confirming no grounds for excluding Contractors from participation in the proceedings are submitted by the Contractor whose offer was the highest rated within the deadline set by the Employer, not shorter than 10 days:

1) informafrom the National Criminal Register in the scope specified in Article 24 (1) (13) and 14 of the PPL (for a person), issued not earlier than 6 months before the deadline for submission of tenders. The Contractor having its registered office in the territory of the Republic of Poland (RP), in relation to a person having a place of residence outside the territory of the Republic of Poland, which concerns the document indicated above, shall submit information from the relevant registerru or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the person who is the information or document concerned resides, to the extent specified in Article 24 paragraph 1 point 13 and 14 of the PPL issued not earlier than 6 months before the deadline for submission of offers. If in the country in which the place of residence is held by a person-the document was to relate, no such documents are issued, it is replaced by a document containing a statement of that person made before a notary or before a judicial or administrative body or a self-governing or economic self-regulatory body competent for the place of residence of that person; 2) information from the National Criminal Register in within the scope specified in Article 24 (1) (21) of the PP (for entitycollective), issued not earlier than 6 months before the deadline for the submission of bids, 3) a statement by the Contractor about the lack of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance contributions, or In the event of the issuance of such a decision or decision, documents confirming the paymentthese receivables together with possible interest or fines or conclusion of a binding agreement regarding the repayment of such receivables, 4) declaration of the Contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders; 5) certificate or manufacturer's certificates offered by the Contractor of the antenna system entitling to its installation and service

3 W terminot 3 days from the date of placing information on the Internet site of the Ordering Party (Article 86 paragraph 5 of the PPA) of a declaration of affiliation or its absence to the same capital group, in accordance with the Act of 6.2.2007 on competition and consumer protection (Journal of Laws of 2015, item 1884, as amended), referred to in Article 24 (1) (23) of the PPA, is submitted by each Economic Operator that submitted an offer.

4 Pursuant to Art. 24aa of the Ordering Party-first evaluate the offers and then examine whether the Contractor, whose offer has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings. Requirements regarding the form of submitted documents are set out in the ToR.

5 Reservation regarding the GDPR according to the content of art. 13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27/04/2016 contains Ch. 21 SIGM.
