The subject of the contract is the provision of agency services for the sale of railway tickets on behalf of and for the benefit of the Employer at the place of the provision of agency ticket sales on Koluszki station, in the premises provided by the Principal.

The subject of the contract is a witnessagency for the sale of railway tickets on behalf of and for the benefit of the Employer at the place of the agency for the sale of tickets at the Koluszki station, in the premises provided by the Principal.

1. Contractors who are not subject to exclusion from the procedure pursuant to art. 24 paragraph 1 point 12 23 (subject to Article 133 paragraph 4 of the Public Procurement Law) and paragraph 5 pkt 1 of the Public Procurement Law,

2. The Contractor is obliged to submit a current statement as at the date of submitting the offers regarding the lack of grounds for exclusion and compliance with the conditions for participation in the procedure. The information contained in the statement constitutes preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The statement referred to in the preceding sentences is made by the Contractorwill give in the form of a single European order document (hereinafter referred to as "EAT"), drawn up in accordance with the specimen contained in a separate attachment "Eat Model", published together with the Terms of Reference on the website: www.platformazakupowa.pl.

3.3. In order to confirm the lack of grounds for excluding the Contractor from participation in the procedure, the Contractor whose offer is chosen as the most advantageous, must submit on requestfor the Ordering Party:

1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders in the proceedings;

2) the Contractor`s statement that he has not issued a final court judgment or the final administrative decision regarding arrears with the payment of taxes, fees or contributions onsocial or health insurance, or in the event of such a judgment or decision being issued, documents confirming the payment of these debts together with any interest or fines, or the conclusion of a binding agreement on the repayment of those debts;

3) the Contractor`s statement that there was no decision against him as a precautionary measure on public procurement;

4) an excerpt from the relevant register or central register and information on business activity, if separate provisions require an entry in the register or in the register to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Public Procurement Law

4. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to inparagraph. 3:

1) point 1 submits information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or the place of residence of the person to whom the information relates, or document, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law;

2) point 4submits a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming that its liquidation has not been opened or the bankruptcy has been declared.

The bid validity period is 60 days. The period begins with the deadline for the submission of tenders.
