Collection and transport services for biological samples and drugs for the transport of blood, urine and bacteriological samples, biopsies, drugs, solutes and hygienic samples for Hospita Centersliers d`Orleans, Pithiviers and Amilly (may also be concerned with the hospitals of Beaugency, Beaune-la-Rolande, Fleury-les-Aubrais, Gien, Neuville-aux-Bois and Sully-sur-Loire).

- lot 1: transport of laboratory biological samples,

- lot 2: transport of medicines from the pharmacy,

- lot 3: transport of grafts,

- lot 4: transport of intra-departmental breast milkental,

- lot 5: transport of labile blood products.

In accordance with article 7 of the consultation regulations, the candidate will have to produce 1 complete file including the documents listed below, sent electronically in accordance with article R. 2132-7 of the public procurement code. The use of the electronic signature is not compulsory. Documents relating to the application:

- letter of application or form DC1 or equivalent, duly completed,

- declaration of the candidate or form DC2 or equivalent, duly completed supplemented with:

—— the list of the main services of the same kind carried out during the last 3 years. This list must show the amount, the period of execution, the names of the principals of the corresponding markets as well as theirrs telephone coordinates (customer references);

2) the overall turnover achieved during the last 3 financial years by indicating the turnover achieved for services of the same type as those being the subject of the contract;

3) the permanent workforce of the company (average annual workforce and size of managerial staff) for each of the last 3 years.
