You can apply for the order-only the Contractor who in accordance with art. 22 ust. 1 point 1 of the PPL are not excluded from the public procurement procedure in the circumstances referred to in art. 24 sec. 1 point 12 23 of the Act. The Contractor shall submit the initial confirmation of the statement in the form of an EAT. The contractor whose offer will be highest assessed will submit: a / current information from the NQF in the scope specified in art. 24 usVol. 1 items 13, 14 and 21 PPL. In the case when the Contractor is established or resides outside the territory of the Republic of Poland, instead of the above-mentioned documents. a - file information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the contractor has its registered office or place of residence or place of residenceboth to which the information or document relates, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law - the Contractor, within 3 days from the date of publishing the report on the opening of tenders, shall provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 Pzp. In order to confirm the fulfillment of the terms related to the subjectthe Contractor will present: 1 Contractor's statement that for the subject matter of the order, the contractor provides 3 cars for transporting patients in a seated position, 2 cars for transporting patients in a lying position, 2 cars with an access ramp for transporting patients on wheelchairs. Contractors may jointly apply for ordering.

In this situation, the Contractor establishesit is the representative (leader) to represent them in the proceedings for the award of the order or representation in the proceedings and conclusion of the public procurement contract and its implementation.
