Main Site: From the settlements in Kardzhali district to "Atanas Dafovski" Hospital, Kardzhali, Belomorski Blvd. "# 53 and vice versa.

The transportation of the patients will be carried out according to a schedule of the number of hemodialysis and shifts attached to the present documentation

Transportation of patients with chronic hemodialysis treatment from their home to a departmenthemodialysis at M. Atanas Dafovski Hospital, Kardjali and vice versa, is carried out from the territory of the following municipalities:

- Kardzhali - 50 patients,

- Chernoochene - 5 patients,

- Djebel - 4 patients,

- Ardino - 3 patients,

- Momchilgrad - 3 patients,

- Kirkovo - 13 patients,

- Krumovgrad - 11 patients

The estimated number of kilometers for a period of 12 months is 560 000 (five hundred and sixtyhundred thousand) km with estimated value of BGN 350, 000.00 (three hundred and fifty thousand leva) without including. VAT. The Assignor will pay the cost of the kilometers traveled only from the home of the hemodialysis patients to the hemodialysis department of "Dr. Atanas Dafovski" Hospital, Kardzhali, and back to their homes.The payment will be for 1 course for 1 hemodialysis change according to the kilometers traveled, but not more than 0, 625 BGN per mile without mileage. VAT and BGN 0.75 per kilometer with VAT included. Less than 1 course is to mean simultaneous transport of patients by 1 hemodialysis (return and return) with 1 or more vehicles to the contractor. Hemodialysis time is not considered a stay and is not paid. The Assignor requires participants to be registered as Merchantsa law entitled to carry out transportation for the public transport of passengers according to the requirements of Art. 6, para. 1 of the Road Transport Act, for which they must hold a valid license for transportation of passengers on the territory of the country, a license for international passenger transport - a Community license issued under Ordinance № 33 of 3.11.1999 on public transport on the roadcargo and cargo on the territory of the Republic of Bulgaria, or a certificate of registration issued in accordance with Ordinance No 34 on Taxi Transfer of Passengers

The participant shall provide information on a valid license for passenger transport on the territory of the Republic of Bulgaria or a license Community or a certificate of registration in Part IV, Section A, point 2 of the EEDOP. The Contracting Authority has no requirements regarding the economic and financial status of the participants. Each participant in the procedure has the right to submit only one offer. The Contracting Authority shall require the Contractor to provide a guarantee for the duration of the Contract in the amount-at 3% of its value, excluding VAT, to ensure its execution. The form of the guarantee may be a sum of money, a bank guarantee or insurance that guarantees performance by covering the contractor's liability. The terms and conditions for the retention or utilization of the guarantee are set out in the draft contract. When submitting a tender, the participant declares the absence of the groundsto remove and comply with the selection criteria by presenting the EEDOP. Where a participant has indicated that it will use subcontractors, a separate EEDOP shall be submitted for each of these persons. Where there is a need to protect personal data or in case of a difference in the circumstances related to the personal condition, the information regarding the requirements of art. 54, para. 1, item 1, item 2 and item 7 of the Public Procurement Act shall be completedin a separate EEDOP for each person or for some of the persons. When examining the tenders, the assignor checks the data requested by the participants, by requiring inf. by other authorities and persons. In the case of unions which are not legal entities, the compliance with the selection criteria shall be demonstrated by the participating association and not by any of the persons included in it.6. A person who participates in a consortium or has consented to be a subcontractor of another participant may not submit a tender on its own. 1 a natural or legal person may participate in only one association. Related parties within the meaning of paragraph 45 of - 2 of the PPA PPL can not be participants in the same procedure. The assignor removes from participation in the participant the procedure in the presence of any hypothesisis listed in Art. 54, para. 1 and Art. 107 of the Public Procurement Act. Required info. shall be stated by the participant in EEDOP, Part III, sections A, B and C. The grounds under Art. 54, para. 1, item 1, item 2 and item 7 shall apply to the persons who represent the participant, the members of the management and the supervisor. organs and others. persons who have the power to exercise control over the decision-making of these bodies. Where the requirements under Art. 54, para. 1, item 1, items 2 and 7 of the PPA refer to more than one person, all persons sign the same EEDOP. Spec. grounds for removal of a participant are the circumstances under:

- Art. 3, item 8 of ЗИФОДЮПДРСС,

- Art. 4, para. 7 in relation to Art. 6, para. 2 of the Measures Against Money Laundering Act and related parties.
