1. The subject of the order is to perform large-scale genotyping in an amount of up to 10, 032 samples for association studiesof the entire genome (GWAS) from patients included in the research in the project entitled "Artificial Intelligence Center in Medicine at the Medical University of Bialystok", implemented by the Medical University of Bialystok.

Main Site: The service will be provided at the Contractor`s premises.

1. large scale genotyping of up to 10, 032 samples for asoc testing is requestedof genomic diseases (GWAS) from patients included in the research in the project entitled: "Artificial Intelligence Center in Medicine at the Medical University of Bialystok", implemented by the Medical University of Bialystok.

2. The detailed description of the subject of the contract and the detailed responsibilities of the Employer and the Contractor include Annex 2 to the ToR - model contract.

3. The Contractor should have the appropriate answerusing equipment to perform large-scale genotyping services using Illinina`s Infinium Global Screening Array-24 panels.

1. Contractors may apply for the contract if they:

- are not subject to exclusion in the absence of grounds for exclusion

Due to failure to meet the conditions referred to in art. 24 paragraph 1 point 12 23 of the Act. The contracting authority does not provide for the possibilityContractor`s exclusion pursuant to art. 24 paragraph 5 of the Act.

- meet the conditions for participation in the proceedings regarding:

1.1. Competence or authority to conduct a specific professional activity, provided that it results from separate provisions, the Employer does not specify the condition,

1.2. economic or financial situation, the Employer does not specify the condition,

1.3. technical or professional ability Employer notdetermines the condition.

The Employer does not specify the condition. the following statements or documents:

1) the Contractor`s declaration made on the basis of art. 25a section 1 clauseawy Pzp, CONCERNING THE EXEMPTIONS FOR EXCLUSION FROM THE PROCEEDINGS, in the form of EAT in accordance with Attachment No. 4 to the ToR;

2) Information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act - issued not earlier than 6 months before the deadline for submission of tenders;

3) the Contractor`s statement that he has not issued a final court judgment or final administrative decisionon arrears in the payment of taxes, fees or social security or health insurance premiums, or in the event of a judgment or decision on documents confirming the payment of such receivables together with any interest or fines, or the conclusion of a binding agreement on the repayment of these receivables,

4) Contractor`s Statement no judgment against him as a measurepreventive prohibition of applying for a public contract,

5) Contractor`s statement on belonging or not belonging to the same capital group; in the case of belonging to the same capital group, the Contractor may submit, together with a statement, documents or information confirming that links with another contractor do not lead to distortion of competition in the proceedings;from art. 26 section 1 of the Act, the Employer will call the Contractor, whose tender is the highest rated, to submit within a specified period, not less than 10 days, valid as at the date of submitting statements and documents confirming the absence of grounds for exclusion.

III. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the document referred to in point II point 2), submits informationit is from an appropriate register, or in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence, the person to whom the information or document relates, in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the expirym of the deadline for submission of tenders.

IV. If in the country in which the Contractor has its registered office or place of residence or place of residence the person to whom the document relates is, the document referred to in point III., It is replaced by a document containing, respectively, a statement of the Contractor with an indication of the person or persons authorized to represent him or a statement of the person whose document wasconcern, submitted to a notary or to a judicial, administrative or professional or economic self-government body competent for the seat or place of residence of the Contractor or the place of residence of that person, issued not earlier than 6 months before the deadline for submission of tenders.

V. Contractor having its seat on the territory of the Republic of Poland, in relation to a person having placese residing outside the territory of the Republic of Poland, to which the document referred to in point II point 2), submits the document referred to in point III. To the extent specified in art. 24 paragraph 1 points 14 and 21 of the Act. If in the country in which the person to whom the document was supposed to reside there does not appear such documents, it is replaced by a document containing the person`s declaration made before a notaryor before a judicial, administrative or professional or economic self-government body competent for the place of residence of that person, issued not earlier than 6 months before the deadline for submitting bids.
