The object of the contract is the selection of 3 experts dealing with the analysis of the financial condition of medical entitiesand indirect cost analysis in the healthcare system, as part of the project entitled "Maps of health needs Database of System and Implementation Analyzes" co-financed by the European Union from the European Social Fund under the Operational Program Knowledge Education Development.

Main Site: The services will be provided at the headquarters of the Employer at 15 Miodowa St. and Nowogr St.clothing 11 in Warsaw or in another place indicated by the Employer in Warsaw.

I .

In order to demonstrate the lack of grounds for exclusion from the procurement procedure in the circumstances of whoreferred to in art. 24 paragraph 1 and art. 24 paragraph 5 points 1, 2, 3, 4 of the Public Procurement Law, the Employer requires the documents referred to in - 5 points 1, 4, 5, 6, 10 in connection with - 7 para. 1 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Employer may request from the Contractor in the procurement procedure.

II.

Amount of deposit required:

Lot 1: PLN 50.00 (in PLN:fifty 00/100),

Lot 2: PLN 50.00 (say: fifty zloty 00/100),

Lot 3: PLN 50.00 (say: fifty zloty 00/100),

the Contractor is obliged to pay the deposit until the deadline for submitting bids set out in the ToR. 45 item 6 of the Public Procurement Law

III.

The Employer provides for the application of the so-called reverse procedureej "in accordance with Article 24aa of the PPL Act

The Employer does not provide for supplementary orders referred to in Article 67 (1) (6) and (7) and Article 134 (6) (3) of the Public Procurement Law

in reference to Section IV. 2.6): "The minimum period during which the bidder will be bound by the offer" The Employer informs that in 2 (two months) 60 days should be taken.

The Employer does not foresee the application of the provisions of art. 29 paragraph 4 paragraphawy Pzp.

V.

Pursuant to art. 93 item 1a of the PPL Act. The contracting authority provides for the possibility of annulment of the procedure for awarding a public contract if the funds which the contracting authority intended to spend on financing all or part of the contract were not awarded to it.

VI.

Of 2018protects personal data and Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 (hereinafter GDPR), has implemented security measures adequate to the risks identified by it and giving the guarantee of proper protection of personal data provided as part of the procedure. Detailed information clause from art. 13 GDPR is contained in the ToR.
