Implementation of the evaluation study entitled: "Comprehensive assessment of the instrument Integrated Territorial Investments under RPOWP 2014 2020"

The subject of the contract is the implementation of the evaluation study: "Comprehensive assessment of the instrumentntu Integrated Territorial Investments under RPOWP 2014-2020 ". The subject of the study is the ITI instrument implemented in the Bialystok Functional Area. The study should cover such a range of data that will allow the full implementation of the objectives of the study. The contractor will propose and justify in the offer the scope of data to be analyzed in a study that will allow to answerall research questions, and thus to achieve the objectives of the study. A detailed description of the subject of the contract is attached as Annex 1 to the ToR. The procedure is conducted under an open tender.

The object of the contract is the implementation of the evaluation study entitled:assessment of the Integrated Territorial Investments instrument under RPOWP 2014-2020 ". In order to confirm the lack of grounds for excluding the Contractor from participation in the proceedings specified in Chapter VIII of the ToR, the Contractor (whose bid was highest rated) will be required to submit: 1) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with payment under, issued not earlier than 3 months before the deadline for submission of bids or requests to participate, or another document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables along with any interest or fines, in particular obtained legal exemption, deferment or extensionpayment in installments of overdue payments or withholding in full of the decision of the competent authority; 2) a certificate from the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 monthsbefore the deadline for submitting bids or requests to participate in the procedure, or another document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption, deferral or distribution provided for by law installments of late payments or withholding in fullthe implementation of the competent authority`s decision; 3) the Contractor`s declaration of non-payment of taxes and local fees, referred to in the Act of 12 January 1991 on local taxes and fees (Journal of Laws of 2019, item 1170, as amended) - the form (template) of this Statement is attached as Annex 7 to the ToR);

4) information from the National Criminal Record in the scope specified in art. 24 paragraph1 point 13, 14 and 21 of the PZP Act, issued not earlier than 6 months before the deadline for submitting bids;

5) the Contractor`s statement that he has not issued a final court judgment or the final administrative decision regarding arrears with the payment of taxes, fees or insurance premiums social or health or - in the event of such a judgment or decision - confirm documentswho pay these debts together with any interest or fines or conclude a binding agreement on the repayment of these debts (the form of the model Declaration is Annex 8 to the ToR); on public procurement (form - model stanowi Annex 9 to the ToR). If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in paragraph 13:

1) point 4 - submits information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place ofapartment or place of residence has a person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 item 13,

14 and 21 of the PZP Act;

2) items 1-2 - submits a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming accordingly that:

a) he is not in arrears with payment of taxes, fees, social security or health insurance contributionsor that it has entered into an agreement with the competent authority on the repayment of these receivables, together with any interest or fines, in particular, it has obtained a legal exemption, deferment or distribution in installments of overdue payments or withholding in full the execution of the decision of the competent authority.

The Purchaser does not formulate inin this respect, the conditions for participating in the proceedings.

1. Each Contractor is obliged to secure a bid bond of PLN 3, 000.00 (say: three thousand zlotys). 2. The bid bond should be lodged before the bid submission deadline. 3. The bid security referred to in para. 1 may be lodged in one or several of the following forms: a) money; b) bank sureties or cooperative suretiessavings and credit, except that guaranteeing the cash register is always a monetary guarantee; c) bank guarantees; d) insurance guarantees; e) sureties granted by entities referred to in art. 6b paragraph 5 point 2 of the Act of November 9, 2000 establishing the Polish Agency for Enterprise Development. 4. The following methods of payment of the bid security are established: a) security in cash -must be transferred to a bank account: Santander Bank Polska S.A. 27 1500 1344 1213 4006 7505 0000 with the note: "Bid bond - implementation of the evaluation study entitled:" Comprehensive assessment of the Integrated Territorial Investments instrument under RPOWP 2014-2020 ", successfully completed before the bid submission deadline (effective deposit payment in cash takes place upon crediting funds) cashon the Purchaser`s bank account before the deadline for submission of bids, i.e. before the day and time set as the deadline for submitting bids - if the bank maintaining the Employer`s account confirms that he has received such a transfer). b) a tender bond in the form of sureties or guarantees. Information clause taking into account the regulations contained in art. 13 of the GDPR, it is included in chapteriale XXVIII SIWZ. The Employer reserves the contractor`s performance of key parts of the contract, i.e.: organization of the contract (study coordination);

2) the implementation of the objectives of the study based on the research methodology approved in the methodological report;

3) analysis of collected data;

4) preparation of the report methodological and final report;

5) cooperation with the Employer, including participation in meetingsand provided for in the Agreement.

The contractor may entrust the performance of part of the subcontractor`s contract. The Employer allows the performance of the subject of the contract with the participation of subcontractors in the scope of the subject of the contract not including key parts of the contract.
