I.

In order to demonstrate the absence of grounds for exclusion from the procurement procedure in the circumstances referred to in art. 24 sec. 1 and art. 24 sec. 5 points 1, 2, 3, 4, 8 of the Public Procurement Law, the Orderer requires dokumentow, referred to in - 5 points 1-6, 9-10, in connection with - 7 para. 1 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Employer may request from the Contractor in the procurement procedure.

The Contractor shall submit in the form of a single document (Single European Order Document - "EAT") a declaration constituting preliminary confirmation that the Contractorit is not subject to exclusion and meets the conditions for participation in the proceedings and meets the selection criteria - art. 25a of the Public Procurement Law

The ordering party must submit a FOOD with a qualified electronic signature issued by the qualified trust service provider.

For encryption signed electronically, EAT is recommended to use the program available on the open-source "7-Zip".

For the timely filed EAT is considered the fact of its receipt on the e-mail server in accordance with the time officially determined by the Central Office of Measures

The amount of the bid bond: PLN 50.00 PLN 00/100).

The Contractor is obliged to pay the bid bond by the deadline for submitting bidsin SIWZ. The Employer allows any form of payment of the bid bond - in accordance with art. 45 par. 6 Public Procurement Law

IV.

The contracting authority provides for the use of so-called the procedure "reversed" in accordance with Article 24aa of the Public Procurement Law - the Employer does not provide for supplementary procurement, referred to in Article 67 paragraph 1 items 6 and 7 and Article 134 paragraph 6 point 3 of the Public Procurement Law.

Referring to the Section IV.2.6): "The minimum period in which the bidt will be bound by the offer "The Contracting Authority informs that for 2 (three months) 60 days should be accepted.

V.

The Contracting Party declares that being the Data Administrator and having regard to the provisions of the Act of 10/05/2018 on the protection of personal data (hereinafter: RODO) ) implemented security measures adequate to the risks identified by it and providing for the proper protection of personal data-transferred as part of the proceeding. A detailed information clause from art. 13 RODO is contained in the SIWZ.
